阑尾系膜电凝法与超声刀在腹腔镜阑尾切除中的应用

Application of Mesenteric Electrocoagulation and Ultrasonic Knife in Laparoscopic Appendectomy

目的:对比腹腔镜阑尾切除术患者中阑尾系膜电凝法与超声刀应用的效果。方法:选取我院普外科2017年3月—2018年8月收治的行腹腔镜阑尾切除术患者60例为对象,根据术中阑尾系膜的处理方法分组:对照组患者应用电凝法,观察组患者应用超声刀,对比两组患者的围术期指标。结果:手术时间、术中出血量、术后住院时间对比,两组差异均无统计学意义, P >0.05;观察组的术后排气时间短于对照组, P <0.05。结论:腹腔镜阑尾切除术中采用超声刀和电凝法处理阑尾系膜均是可行的,均能顺利完成,但是相较于电凝法,超声刀处理系膜具有快速简便、适用范围广等优点,更有利于患者术后尽早排气,值得推广。

Objective: To compare the effect of mesenteric electrocoagulation and ultrasonic scalpel in laparoscopic appendectomy. Methods: 60 cases of laparoscopic appendectomy treated in general surgery department of our hospital from March 2017 to August 2018 were selected as objects. According to the treatment of mesentery during operation, patients in the control group were treated with electrocoagulation, while patients in the observation group were treated with ultrasonic scalpel. Results: There was no significant difference between the two groups in operation time, intraoperative bleeding volume and postoperative hospitalization time( P >0.05), and the postoperative exhaust time of the observation group was shorter than that of the control group( P <0.05). Conclusion: Ultrasound scalpel and electrocoagulation are both feasible and successful in laparoscopic appendectomy, but compared with electrocoagulation, ultrasound scalpel treatment of mesentery has the advantages of fast, simple and wide application, which is more conducive toearly exhaust after operation, which is worthy of promotion.
