鼻内镜下治疗鼻息肉后患者嗅觉恢复影响因素分析

Influencing factors of olfactory recovery in patients with nasal polyps after treatment under nasal endoscope

目的 探讨鼻内镜下治疗鼻息肉后患者影响患者嗅觉恢复相关因素。方法 选取2008年1月至2018年3月本院鼻内镜下治疗鼻息肉患者116例作为研究对象,根据患者术后3月嗅觉恢复情况分为恢复组(91例)和未恢复组(25例),收集两组患者一般临床资料和可疑危险因素,采用单因素和多因素Logistic回归分析影响患者术后嗅觉恢复危险因素。结果 单因素分析得出,吸烟、过敏性鼻炎、鼻中隔偏曲、鼻息肉直径、术后并发症在两组患者中比较差异有统计学意义(P<0.05)。多因素分析得出,过敏性鼻炎(OR=3.647,95%CI:1.425~16.547,P<0.05)和术后并发症(OR=2.226,95%CI:1.181~12.147,P<0.05)为影响鼻内镜下治疗鼻息肉患者嗅觉恢复危险因素。结论 过敏性鼻炎病史、术后并发症为影响鼻内镜治疗鼻息肉患者术后嗅觉功能恢复危险因素,临床中针对性进行干预和预防,对保障患者嗅觉功能有重要临床意义。

Objective To explore the factors influencing the recovery of the smell after endoscopic treatment for nasal polyps. Methods 116 patients with nasal polyps taking endoscopic treatment in our hospital from Jan., 2008 to Mar., 2018 were selected as study objects, and were divided into a recovery group (91 cases) and an unrecovered group (25 cases). The general clinical data and suspicious risk factors were collected from the two groups. Univariate and multivariate logistic regression analysis were used to analyze the risk factors for postoperative olfactory recovery. Results Univariate analysis showed that there were statistical differences in smoking, allergic rhinitis, nasal septum deviation, nasal polyp diameter, and postoperative complications between the two groups (all P < 0.05). Multivariate analysis showed that allergic rhinitis (OR=3.647, 95%CI: 1.425- 16.547, P < 0.05) and postoperative complications (OR=2.226, 95% CI: 1.181-12.147, P < 0.05) were olfactory risk factors in patients with nasal polyps taking endoscopic treatment. Conclusion The history of allergic rhinitis and postoperative complications are the risk factors for the recovery of postoperative olfactory function in patients with nasal polyps treated by nasal endoscopy. The targeted intervention and prevention in clinical practice have important clinical significance for ensuring the olfactory function of patients.
