利培酮对首发精神分裂症患者神经营养因子及认知功能影响 被引量:4

Effect of risperidone on neurotrophic factors and cognitive function in patients with first-episode schizophrenia

摘要 目的:研究利培酮对首发精神分裂症患者血清脑源性神经营养因子(BDNF)、胶质源性神经营养因子(GDNF)的表达变化及认知功能影响。方法:收集首发精神分裂症患者56例,给予利培酮治疗16周,剂量为(4.23±1.35)mg/d。比较治疗前后患者临床症状、认知功能和血清神经营养因子表达变化,并对血清BDNF、GDNF水平与认知功能评分进行相关性分析。结果:与治疗前比较,治疗后患者PANSS量表评分显著降低,差异有统计学意义(P<0.05)。患者血清BDNF和GDNF表达水平较治疗前显著提高,且临床疗效越好,血清BDNF和GDNF表达水平越高,差异具有统计意义(P<0.05)。与治疗前比较,患者的认知功能各指标有所改善,差异有统计学意义(P<0.05)。治疗前BDNF、GDNF浓度与认知功能各指标均有显著相关性,差异有统计学意义(P<0.05),治疗后与认知功能各指标均无相关,差异有统计学意义(P<0.05)。结论:利培酮治疗能够显著提高首发患者血清BDNF和GDNF水平,改善患者认知功能和精神症状,检测血清BDNF、GDNF水平有助于精神分裂症的病情判断。 Objective To investigate the effects of risperidone on the expression of serum brain-derived neurotrophic factor (BDNF) and glial-derived neurotrophic factor (GDNF) and cognitive function in patients with first-episode schizophrenia. Method A total of 56 patients with first-episode schizophrenia admitted were treated with risperidone for 16 weeks at a dose of (4.23±1.35) mg/d.Compared the changes of clinical symptoms,cognitive function and serum neurotrophic factor expression before and after treatment.Correlation analysis was performed between serum BDNF and GDNF levels and cognitive function scores. Results Compared with before treatment,the PANSS score of the patients after treatment was significantly lower ( P <0.05).The expression levels of BDNF and GDNF in patients were significantly higher than those before treatment,and the better the clinical efficacy,the higher the expression levels of serum BDNF and GDNF,the difference was statistically significant ( P <0.05).Compared with before treatment,the cognitive function of the patients improved ( P < 0.05).The concentrations of BDNF and GDNF were significantly correlated with the cognitive function before treatment ( P <0.05).There was no correlation between the treatment and cognitive function ( P <0.05). Conclusion Risperidone treatment can significantly improve the serum levels of BDNF and GDNF in the first-time patients,improve cognitive function and psychiatric symptoms,and detect serum BDNF and GDNF levels to help determine the condition of schizophrenia.
