心肌酶在自发性蛛网膜下腔出血与创伤性蛛网膜下腔出血中的水平研究 被引量:1

摘要 目的评价心肌酶在自发性蛛网膜下腔出血(Subarachnoid hemorrhage,SAH)和创伤性SAH患者中的血清水平。方法对入院的SAH患者进行CT及腰椎穿刺检查,并经医师诊断筛选出自发性SAH患者和创伤性SAH患者各45例做为观察组,另选同期在深圳市宝安区中心医院进行体检的健康者90例做为对照组,检测两组患者入院24h内的心肌酶谱,即谷草转氨酶(AST)、肌酸激酶(CK)和乳酸脱氢酶(LDH)的血清水平,并进行统计分析。结果对观察组和对照组患者的血清AST、CK、LDH水平进行统计分析,观察组患者的AST、CK、LDH的血清水平显著高于对照组,差异有统计学意义(P<0.05);其中自发性SAH患者的AST、CK、LDH的血清水平显著高于创伤性SAH患者,差异有统计学意义(P<0.05)。结论自发性SAH患者的心肌酶谱血清水平显著高于创伤性SAH患者,对于自发性SAH和创伤性SAH的并发症及发病机制有进一步的理解,更好的减少治疗期间的并发症发生,提高医疗质量。 Objective To evaluate the serum level of myocardial enzymes in patients with spontaneous subarachnoid hemorrhage and traumatic subarachnoid hemorrhage.Methods The patients with subarachnoid hemorrhage were examined by CT and lumbar puncture,inclading 45 cases of patients with spontaneous subarachnoid hemorrhage and 45 cases of patients with traumatic subarachnoid hemorrhage who screened out by physician’s diagnosis were selected as the observation group.Another 90 cases of healthy persons who underwent physical examination in the hospital during the same period were selected as the control group.Then,the serum levels of myocardial enzymes,ie.Glutamic oxaloacetic transaminase(AST), creatine kinase(CK) and lactate dehydrogenase (LDH))of two groups were measured and statistically analyzed within 24 hours after admission.Results The serum AST, CK and LDH levels in the observation group and the control group were statistically analyzed.The serum levels of AST,CK and LDH in the observation group were significantly higher than those in the control group(P< 0.05).The serum levels of AST,CK and LDH in patients with spontaneous subarachnoid hemorrhage were significantly higher than those in patients with traumatic subarachnoid hemorrhage.There was statistical difference(P< 0.05).Conclusion The serum levels of myocardial enzymes in patients with spontaneous subarachnoid hemorrhage are significantly higher than those in patients with traumatic subarachnoid hemorrhage.Further understanding of the complications and pathogenesis of spontaneous subarachnoid hemorrhage and traumatic subarachnoid hemorrhage can better reduce complications during treatment,and improve medical quality.
