急诊早期急性重症有机磷农药中毒致呼吸衰竭应用呼吸机治疗的效果探究

Effect of Ventilator Therapy on Respiratory Failure Caused by Acute Severe Organophosphorus Pesticide Poisoning in the Early Stage of Emergency

目的探讨呼吸机应用在急诊早期急性重症有机磷农药中毒致呼吸衰竭治疗中的临床价值。方法以洛阳市中心医院急诊科接收的急性重症有机磷农药中毒致呼吸衰竭患者(65例)为观察目标,对其在常规治疗的基础上应用呼吸机,对患者的治疗成效予以评价。结果65例患者住院用时(9.17±1.14)d,意识恢复用时(7.35±0.87)h,自主呼吸恢复用时(7.11±0.64)h;治疗前后血液酸碱度(pH)、动脉血二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)各指标均得到显著改善;有33例(94.29%)患者痊愈,有2例(5.71%)患者死亡,其中,因多器官功能衰竭、呼吸及循环衰竭死亡各1例。结论将呼吸机应用于急诊早期急性重症有机磷农药中毒致呼吸衰竭治疗中,可达到突出治疗效果,挽救患者生命。

Objective To investigate the clinical value of ventilator in the treatment of respiratory failure caused by acute severe organophosphorus pesticide poisoning in early emergency.Methods 65 patients(65 cases)with respiratory failure caused by acute severe organophosphorus pesticide poisoning received in Emergency Department of the hospital were treated with ventilator on the basis of routine treatment,and the therapeutic effect was evaluated.Results The duration of hospitalization was(9.17±1.14)days,the time of consciousness recovery was(7.35 ±0.87)h,the time of spontaneous respiration was(7.11±0.64)h.The blood pH of(pH),partial pressure of carbon dioxide(PaCO2)and partial pressure of oxygen in arterial blood(PaO2)were significantly improved.33 cases (94.29%)were cured,2 cases(5.71%)died,among them,1 case died from multiple organ failure,1 case from respiratory failure and 1 case from circulatory failure.Conclusion The application of ventilator in the treatment of respiratory failure caused by acute organophosphorus pesticide poisoning in the early stage of emergency can achieve outstanding therapeutic effect and save patients’lives.
