早期功能锻炼对肘部骨折患者内固定术后肘关节功能的影响

Effect of Early Functional Exercise on Elbow Joint Function of Patients with Elbow Fracture

目的研究早期功能锻炼对肘部骨折患者术后肘关节功能的影响。方法选取2017年2月—2018年2月菏泽市中医医院骨科收治的52例肘部骨折患者为研究对象。按随机数表法分为研究组、对照组,每组各26例。研究组术后即指导患者进行功能锻炼,对照组于术后3周末开始进行骨科常规康复训练。所有研究对象均获随访,随访时间6~12个月,中位数9个月。术后3个月根据FLynn肘关节功能评分评定治疗效果;通过影像学检查,观察记录患者骨折愈合时间;采用Barthel指数评分对患者日常生活活动能力进行评估。结果锻炼后,研究组优良率(92.3%)显著高于对照组(75.0.%)(P<0.05);研究组在肘关节屈曲度、伸直度、前臂旋前度、前臂旋后度均显著优于对照组(P<0.05)。所有患者骨折均愈合,研究组愈合平均时间为(11.3±2.5)周,明显优于对照组愈合平均时间(12.7±2.6)周(P<0.05)。结论肘部骨折术后患者早期功能锻炼能够促进骨折愈合和术后关节功能恢复,从远期疗效来看具有临床推广和运用价值。

Objective To study early functional exercise on elbow joint function of patients with elbow fracture.Methods 52 cases of elbow fracture in the hospital from February, 2017 to February, 2018 were selected.According to the method of random number,they were divided into the study group and the control group,with 26 cases in each group.The patients in the study group received early functional exercise,while the patients in the control group received routine orthopaedic rehabilitation training at the end of 3 weeks after operation.All subjects were followed up for 6~12 months with a median of 9 months.Three months after surgery, FLynn elbow function score was used to evaluate the therapeutic effect,the fracture healing time was recorded by imaging examination, and the activity of daily living(ADL)was evaluated by Barthel index.Results After exercise, the excellent and good rate in the study group(92.3%)was significantly higher than that in the control group(75.0%)(P <0.05).The elbow flexion,extension,forearm pronation and supination in the study group were significantly better than the control group(P <0.05).The average healing time in the study group was(11.3 ±2.5)weeks,which was significantly better than(12.7 ±2.6)weeks in the control group (P <0.05).Conclusion The early functional exercise on elbow joint function of patients with elbow fracture can promote the healing of fracture and the recovery of joint function.From the point of view of long-term effect, it has clinical promotion and application value.
