颈椎自稳型椎间融合器在颈椎前路减压融合内固定术的应用

Application of stand-alone MC-PEEK cage in anterior cervical discectomy with fusion internal fixation

目的观察颈椎自稳型椎间融合器在颈椎前路减压融合内固定术(ACDF)的应用效果。方法将26例行单节段ACDF治疗的颈椎病患者分成A、B两组,A组使用颈椎自稳型椎间融合器,B组使用颈椎前路钛板联合颈椎椎间融合器。对两组患者术前和术后3d、1年、3年、5年颈椎融合节段上下椎体前、后缘高度变化(HAB、HPB)、颈椎曲度改变(D值、Cobb角)进行统计分析。结果患者均获得随访,时间5~6(5.4±0.7)年。A、B两组患者术后3d、1年、3年、5年颈椎融合节段上下椎间隙高度、颈椎曲度较术前明显改善(P<0.05),两组各相同时间点颈椎融合节段上下椎间隙高度、颈椎曲度比较差异无统计学意义(P>0.05)。结论应用颈椎自稳型椎间融合器可获得较好的近、中期疗效。

Objective To observe the application effect of stand-alone MC-PEEK cage in anterior cervical discectomy with fusion(ACDF).Methods The 26 patients with single segment cervical spondylosis treated with ACDF were divided into two groups.Group A was treated with stand-alone MC-PEEK cage,and group B was treated with anterior cervical titanium plate combined with cervical interbody fusion cage.The changes of anterior and posterior edge height(HAB,HPB)and cervical curvature(D value,Cobb angle)of fusion segment were statistically analyzed at the preoperation,3 d,1,3 and 5 years after operation in both groups.Results All the patients in both groups were followed up for 5~6(5.4±0.7)years.The height of upper and lower cervical intervertebral space of fusion segment and the degree of physiological curvature were significantly improved in group A and B at 3 d,1,3 and 5 years after operation(P<0.05).There were no significant differences in the height of upper and lower cervical intervertebral space and the degree of cervical curvature of cervical fusion segment between the two groups at the same time points(P>0.05).Conclusions The stand-alone MC-PEEK cage can obtain better short-medium term clinical effect.
