骨水泥型与生物型髋关节假体置换后对老年骨质疏松患者凝血功能的影响 被引量:5

摘要 目的观察骨水泥型与生物型髋关节假体置换后对老年骨质疏松患者凝血功能的影响。方法按照患者选择的不同假体将72例需行全髋关节置换(THA)的老年骨质疏松患者分为骨水泥组(38例)和生物假体组(34例),比较两组患者术后24h血浆凝血酶原时间(PT)、活化的部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)及血浆D-二聚体(D-D)水平的变化。结果术后24h两组各项指标基本趋于正常,差异无统计学意义(P>0.05)。术后24h骨水泥组D-D有升高趋势,但与生物假体组比较差异无统计学意义(P>0.05)。结论老年骨质疏松患者行THA采用骨水泥型与生物型假体治疗后凝血功能无显著差异。 Objective To observe the effect of bone cement and biological prosthesis hip arthroplasty on coagulation function in elderly patients with osteoporosis.Methods The 72 elderly patients with osteoporosis requiring artificial total hip arthroplasty(THA)were divided into bone cement group(38 cases)and bioprosthesis group(34 cases)according to different prostheses selected by the patients.Changes in plasma prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(FIB)and plasma D-dimer(D-D)levels at 24 h after surgery were compared.Results The indexes of the two groups were basically normal at 24 h after operation,and the difference was not statistically significant(P>0.05).At 24 h postoperation,there was an increasing trend of D-D in the bone cement group,but the difference was not statistically significant(P>0.05),compared with the bioprosthesis group.Conclusions There is no difference in coagulation function for bone cement prosthesis and bioprosthesis undergoing THA in the treatment of elderly patients with osteoporosis.
