摘要
目的探讨糖尿病前期(impaired glucose regulation,IGR)人群中血清25羟维生素D 3[25(OH)D 3]水平的变化情况及其相关因素。方法依据1999年WHO糖尿病专家委员会报告提出的分类标准,按照口服葡萄糖耐量(oral glucose tolerance test,OGTT)结果,选取2017至2018年来院体检的在职公务员为研究对象,入组156例分为IGR组71例,健康对照(normal control,NC)组85例,检测并比较2组一般临床生化指标和血清25(OH)D 3水平的差异,并采用Pearson相关分析法评估血清25(OH)D 3与上述指标间的关系。结果糖尿病前期组体重指数(BMI)、腰臀比(WHR)、收缩压(SBP)、舒张压(DBP)、胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、餐后2 h血糖(2 hPG)、尿酸(UA)、C反应蛋白(CRP)水平高于健康对照组,差异均有统计学意义(P<0.05)。HDL-C、25(OH)D 3水平低于健康对照组,差异均有统计学意义(P<0.05)。2组FPG水平比较,差异无统计学意义(P>0.05)。IGR组血清25(OH)D 3水平低于健康对照组,差异有统计学意义(P<0.05);血25(OH)D 3水平与年龄、BMI、WHR、SBP、2 hPG、TC、LDL-C、UA、CRP水平呈负相关(r值分别为-0.47、-0.35、-0.17、-0.12、-0.26、-0.25、-0.20、-0.22、-0.27,P<0.05);与HDL-C呈正相关(r=0.33,P<0.05),与DBP、TG、FPG未见显著相关。IGR组中血清25(OH)D 3不足和缺乏者较健康对照组显著增加,差异有统计学意义(P<0.05)。结论血25(OH)D 3水平在糖尿病前期人群明显降低,提示其或可成为糖尿病前期的重要观察指标之一。
Objective To investigate the changes of serum levels of 25-hydroxy vitamin D[25(OH)D 3]in prediabetic state population(impaired glucose regulation,IGR)and related factors.Methods According to the WHO standard of diabetes separate criteria in 1999 and the examination results of oral glucose tolerance test(OGTT),among the civil servants on the job who underwent physical examination from 2017 to 2018,156 persons were enrolled in the study,who were divided into IGR group(n=71)and normal control(NC)group(n=85).The differences of clinical biochemical indicator and the serum levels of[25(OH)D 3]were observed and compared between the two groups.Moreover Pearson correlation analysis was used to evaluate the the correlatin between[25(OH)D 3]and above indexes.Results The levels of BMI,WHR,SBP,DBP,TC,TG,LDL-C,2 hPG,UA,CRP in IGR group were significantly higher than those in NC group(P<0.05).The serum levels of[25(OH)D 3]in IGR group were significantly lower than those in NC group(P<0.05).The serum levels of 25(OH)D 3]were negatively correlated with patient’s age,BMI,WHR,SBP,2h BG,TC,LDL-C,UA and CRP(P<0.05),however,which were positively correlated with those of HDL-C(P<0.05),moreover,which were not correlated with those of DBP,TG and FPG.Conclusion The serum levels of[25(OH)D 3]in prediabetic state population are significantly decreased,which suggests that it may be one important observation indexes to forecast prediabetic state.
