左炔诺孕酮治疗子宫内膜增生症的效果及作用机制研究 被引量:13

摘要 目的分析左炔诺孕酮治疗子宫内膜增生症的临床有效性与作用机制。方法收集子宫内膜增生症患者156例,随机分为2组,每组78例。试验组给予左炔诺孕酮宫内缓释系统治疗,对照组给予醋酸甲羟孕酮口服治疗,比较2组患者治疗效果。观察2组治疗后月经评分、子宫内膜厚度,采用免疫组化方法检测雌、孕激素受体含量变化情况。结果治疗后2组患者经量及子宫内膜厚度均改善,其中试验组改善明显,差异有统计学意义(P<0.05);与对照组比较,试验组患者子宫内膜腺体、间质中雌激素受体、孕激素受体含量较低,差异有统计学意义(P<0.05)。结论左炔诺孕酮宫内缓释系统对子宫内膜增生症患者治疗效果显著,能有效改善患者月经紊乱及子宫内膜厚度,具有较好的临床应用价值及意义。 Objective To investigate the clinical efficacy and action mechanism of levonorgestrel in treatment of endometrial hyperplasia.Methods A total of 156 patients with endometrial hyperplasia were randomly divided into experimental group and control group,with 78 cases in each group.The patients in control group were treated by oral medroxyprogesterone acetate tablets,however,the patients in experimental group were treated by levonorgestrel intrauterine delivery system.After treatment the clinical efficacy,menstrual score and endometrial thickness were observed and compared between the two groups.Moreover the changes of estrogen and progesterone receptor levels were detected by immunohistochemistry.Results After treatment,the menstrual volume and endometrial thickness were improved in both groups,and the improvement degree in experimental group was more significant than that in control group(P<0.05).In addition the levels of endometrial gland,interstitial estrogen receptor,progesterone receptor in experimental group were significantly lower than those in control group(P<0.05).Conclusion The application of levonorgestrel intrauterine delivery system in treatment of endometrial hyperplasia can obtain obvious therapeutic effects,which can improve the menstrual disorder and endometrial thickness of patients,therefor,it is worthy of clinical application and promotion.
