精细化护理对腹会阴联合直肠癌根治术患者的护理效果研究

Clinical nursing effects of refined nursing for patients with rectal cancer after abdominal perineal resection combined with radical resection of rectal cancer

目的评价精细化护理对腹会阴联合直肠癌根治术患者术后康复效果的影响。方法选取2017年8月至2018年8月接受腹会阴联合直肠癌根治术治疗的直肠癌患者68例,根据采用的护理方法不同分为研究组和对照组。研究组采取精细化护理,对照组采用常规护理。评价2组患者住院及并发症相关情况,统计2组患者活动能力、焦虑、抑郁及对护理满意度评分情况。结果研究组患者导尿管滞留时间、术后进食时间及平均住院时间均明显少于对照组(P=0.000;P=0.006;P=0.001),研究组患者出现会阴部切口感染及并发症的总发生率均低于对照组(P=0.038;P=0.001),研究组患者Barthel指数评分良、焦虑(SAS)评分轻度、抑郁(SDS)评分的比例均明显高于对照组(P=0.016;P=0.013;P=0.012),研究组患者Barthel指数评分差、重度的比例均低于对照组(P=0.043;P=0.031),研究组患者十分满意、不满意的比例及护理满意率与对照组相比较差异均具有有统计学意义(P=0.046;P=0.032;P=0.019)。结论精细化护理策略可以提高腹会阴联合直肠癌根治术患者的术后康复效果、护理满意度及活动能力,降低焦虑及抑郁的情况。

Objective To evaluate the effects of refined nursing on postoperative rehabilitation of patients with with rectal cancer after abdominal perineal resection combined with radical resection of rectal cancer.Methods A total of 68 patients with rectal cancer who underwent abdominal perineal resection combined with radical resection in our hospital from August 2017 to August 2018 were enrolled in the study.These patients were divided into research group and control group according to different nursing methods.The patients in research group were treated by refined nursing,however,the patients in control group were treated by routine nursing.The hospitalization duration and the incidence of complications were observed and compared between the two groups.Moreover the questionnaires were used to evaluate the activity ability,anxiety and depression status,nursing satisfaction degree of patients.Results The catheterization retention time,postoperative feeding time and average hospitalization time in research group were significantly lower than those in control group(P<0.01).The total incidence rates of perineal wound infection and complications in research group were significantly lower than those in control group(P<0.01).moreover the Barthel index score,mildness of anxiety(SAS)score,and depression(SDS)scores in research group were sueprior to those in control group(P<0.05).There were significant differences in the proportion of patients with satisfaction and dissatisfaction as well as the nursing satisfaction rates between the two groups(P<0.05).Conclusion The refined nursing strategy can improve the postoperative rehabilitation effects in patients with with rectal cancer after abdominal perineal resection combined with radical resection of rectal cancer,which can improve nursing satisfaction degree and activity ability of patients and can relieve anxiety and depression status of patients.
