青海大学附属医院70例重症肌无力患者生活质量现状调查

Investigation and Analysis of Quality of Life in 70 Cases with Myasthenia Gravis in the Affiliated Hospital of Qinghai University

目的调查重症肌无力门诊患者生活质量现状及相关影响因素。方法选取2017-06/2018-04在青海大学附属医院神经内科接受门诊复查的重症肌无力患者,应用重症肌无力量化评分(QMGs)评估患者病情严重程度,15项重症肌无力生活质量(MG-QOL15)量表评估生活质量现状,采用Logistic回归模型分析影响生存质量的相关因素。结果 70例重症肌无力患者总MG-QOL15评分为(25.15±10.96)分,第15问题项目MG-QOL15评分最低为(0.88±1.21)分,第2问题项目MG-QOL15评分最高为(2.05±1.23)分;不同年龄、文化程度、病情严重程度、医疗保险类型MG-QOL15评分存在差异(P<0.05);Logistic回归模型分析显示:年龄(≥40岁)、文化程度(高中及以下)、病情严重程度(≥20分)、医疗保险类型(无社会医疗保险)是影响重症肌无力患者生存质量的独立危险因素(P<0.001)。结论重症肌无力门诊患者生存质量水平较低,年龄、病情严重程度、无社会医疗保险是影响重症肌无力患者生存质量的危险因素,需要进一步完善卫生服务及医疗保障体系,提高重症肌无力患者生存质量。

Objective To investigate the status of quality of life and related influencing factors in outpatients with myasthenia gravis(MG). Methods The MG patients admitted to the Affiliated Hospital of Qinghai University from June 2017 to April 2018 were investigated. The severity of MG was assessed by QMGs and the quality of life was assessed by MG-QOL15 scales. Logistic regression model was used to analyze the related factors affecting the quality of life. Results The total MG-QOL15 score of 70 MG patients with myasthenia gravis was(25.15 ± 10.96). The lowest MG-QOL15 score was(0.88 ± 1.21) in Question 15, and the highest MG-QOL15 score was(2.05 ± 1.23) in Question 2. There were significant differences in MG-QOL15 scores among different age, education level, severity of illness and types of medical insurance(P<0.05). Logistic regression model analysis showed that age(>40 years old),education level(high school and below),severity of illness(>20 points),type of medical insurance(without social medical insurance) were independent risk factors affecting the quality of life of MG patients(P<0.001). Conclusion The quality of life of MG outpatients is low. Age, severity of illness and lack of social medical insurance are risk factors affecting the quality of life of MG patients. Health service and medical security system need to be further improved to improve the quality of life of MG patients.
