摘要
目的观察补肾活血方治疗膝关节骨性关节炎(KOA)的临床疗效及分子机制。方法92例KOA患者,采用随机数字表法分为治疗组和对照组,每组46例。治疗组患者给予补肾活血方治疗,对照组患者给予硫酸氨基葡萄糖片治疗。比较两组患者临床疗效、不良反应发生情况及治疗前后白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)及一氧化氮(NO)水平。结果治疗组患者显效率19.6%、有效率47.8%高于对照组的4.3%、26.1%,差异具有统计学意义(P<0.05);治疗组总有效率高于对照组,但差异无统计学意义(P>0.05)。治疗前,两组患者血清NO、IL-6、TNF-α水平比较,差异无统计学意义(P>0.05);治疗2、4周后,两组患者血清NO、IL-6、TNF-α水平均低于本组治疗前,且治疗组各指标水平均明显低于对照组,差异具有统计学意义(P<0.05)。两组患者不良反应发生率比较差异无统计学意义(P>0.05)。所有患者均获随访,均无脱落。结论补肾活血方可能通过降低KOA患者IL-6、TNF-α及NO等血清炎症指标水平以缓解疼痛,改善膝关节功能。
Objective To observe the clinical efficacy of Bushen Huoxue prescription in the treatment of knee osteoarthritis(KOA)and its molecular mechanism.Methods A total of 92 KOA patients were divided into treatment group and control group according to random number-table method,with 46 cases in each group.The treatment group was treated with Bushen Huoxue prescription,and the control group was treated with glucosaminesulfate tablets.Comparison were made on clinical efficacy,occurrence of adverse reactions,interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and nitric oxide(NO)before and after treatment in two groups.Results The treatment group had higher excellent rate as 19.6%and effective rate as 47.8%than 4.3%and 26.1%in the control group,and their difference was statistically significant(P<0.05).The treatment group had higher total effective rate than the control group,but the difference was not statistically significant(P>0.05).Before treatment,both groups had no statistically significant difference in serum NO,IL-6 and TNF-α(P>0.05).After 2 and 4 weeks of treatment,both groups had lower serum NO,IL-6 and TNF-αthan those before treatment,and the treatment group had obviously lower indicators than the control group.Their difference was statistically significant(P<0.05).Both groups had no statistically significant difference in incidence of adverse reactions(P>0.05).All patients were followed up without falling off.Conclusion Bushen Huoxue prescription may alleviate pain and improve knee joint function by reducing the levels of serum inflammatory markers of IL-6,TNF-αand NO in KOA patients.
