医患矛盾:基于制度失范的体系化解释

Doctor-patient Contradiction: A Systematic Explanation Based on Institutional Anomie

医患矛盾不仅干扰了正常的社会秩序,也严重削弱了政府公信力。医患矛盾与医患之间的信息不对称相关,但医院、医生、医保相关的管理体制与机制对医患关系能够产生重要影响。通过财政体制、药品流通体制、医院管理体制、医疗保险制度及惩戒制度五个维度进行分析,分别阐述了其与医患矛盾之间的深层次关系,着重指出了医疗相关制度对医患矛盾的影响机制。医患矛盾也必须在三医联动的背景下,通过深化管理体制的改革才能进一步缓解。

Doctor-patient contradiction not only disturbs the normal social order, but also seriously weakens the credibility of the government. Doctor-patient contradiction is related to information asymmetry between doctors and patients. Besides, the management system and mechanism related to hospitals, doctors and medical insurance have an important impact on doctor-patient relationship. Through the analysis of five dimensions of financial system, drug circulation system, hospital management system, medical insurance system and disciplinary system, this paper expounds the deep-seated relationship between institutional anomie and doctor-patient contradiction, emphatically points out the influence mechanism of medical related system on doctor-patient contradiction. Only by deepening the joint reform of medical care system, medical insurance system and drug administration system can we further relief the contradiction between doctors and patients.
