CK5/6、CK7以及CYFRA21-1联合检测在肺腺癌骨转移的诊断研究

Combined detection of CK5/6 ,CK7 and CYFRA21-1 in the diagnosis of bone metastasis from lung adenocarcinoma

目的 探讨细胞角蛋白家族细胞角蛋白5/6(CK5/6)、CK7以及细胞角蛋白19片段(CYFRA21-1)联合检测在肺腺癌骨转移的诊断研究.方法 纳入2015年6月至2018年6月咸宁市中心医院收治的肺腺癌骨转移患者和肺腺癌非骨转移患者各60例,分为转移组和非转移组.酶联免疫吸附试验检测2组患者血清中CK5/6、CK7以及CYFRA21-1的表达量.采用受试者工作特征曲线分析CK5/6、CK7以及CYFRA21-1检测在肺腺癌骨转移中的诊断意义.结果 转移组血清中CK5/6的表达量低于非转移组(t=10.43,P<0.05),转移组血清中CK7以及CYFRA21-1的表达量均高于非转移组(t=17.43、16.79,P值均<0.05).转移组和非转移组的CK5/6、CK7以及CYFRA21-1的敏感度分别为77% 、77% 、80%,特异度分别为77% 、83% 、87%;CK5/6、CK7以及CYFRA21-1联合检测对肺腺癌骨转移诊断的敏感度为77.05%,特异度为77.97%,准确度为77.50%.结论 血清中CK5/6、CK7以及CYFRA21-1表达量的检测可作为诊断肺腺癌骨转移的潜在标志.

Objective To investigate the diagnostic study of keratinocyte family cytokeratin 5/6 ( CK5/6 ) ,CK7 and cytokeratin 19 fragment ( CYFRA21-1 ) in bone metastasis of lung adenocarcinoma .Methods The 60 patients with bone metastases from lung adenocarcinoma and the 60 patients with non-bone metastasis from lung adenocarcinoma were enrolled in Xianning Central Hospital ,the First Affiliated Hospital of Hubei University of Science and Technology .They were divided into metastasis group and non-metastasis group .The enzyme-linked immunosorbent assay was used to detect the expressions of CK5/6 ,CK7 and CYFRA21 in serum of two groups . The diagnostic significance of CK5/6 , CK7 and CYFRA21-1 in lung metastasis of lung adenocarcinoma was analyzed by receiver operating characteristic curve .Results The expression of CK5/6 in serum of patients with bone metastases from lung adenocarcinoma was lower than that of non-bone metastasis of lung adenocarcinoma ( t =10 .43 , P <0 .05) ,while the expression of CK7 and CYFRA21-1 in serum of patients with bone metastases from lung adenocarcinoma was higher than that of non-bone metastasis of lung adenocarcinoma ( t =17 .43 ,16 .79 ,both P <0 .05);the sensitivity of CK5/6 ,CK7 and CYFRA21-1 in metastatic group and non-metastatic group were 77% ,77% ,80% ,respectively ,and the specificity was 77% ,83% ,87%;the sensitivity of CK5/6 , CK7 and CYFRA21-1 in the diagnosis of bone metastasis of lung adenocarcinoma was 77 .05% ,the specificity was 77 .97% ,and the accuracy was 77 .50% .Conclusions The detection of CK5/6 ,CK7 and CYFRA21-1 in serum can be used as a potential marker for the diagnosis of bone metastasis in lung adenocarcinoma .
