临床药师对重症监护室住院患者198例医嘱干预情况分析

Analysis of the intervention of 198 intensive care unit patients by clinical pharmacists

目的分析临床药师对重症监护室(ICU)不合理用药医嘱的干预情况.方法按照用药干预发生时间、不合理用药类型及不合理药物种类统计分析医院2017年5月-2018年6月在临床查房与医嘱审核过程中发现的不合理并成功干预的用药医嘱.结果临床药师干预不合理用药医嘱198例,其中干预时间多发生在星期一(33.84%),不合理用药类型以用法用量不适宜较多(63.13%),不合理药物种类以抗菌药物居多(52.58%).结论临床药师的用药医嘱干预为保证用药安全有效发挥重要作用,有利于促进合理用药,提高临床药师服务质量.

Objective To analyze the intervention of clinical pharmacists on irrational medication orders in adult in- tensive care unit( ICU). Methods According to the time of drug intervention,the type of irrational drug,and the type of irra- tional drug,to analyze the unreasonable and successful intervention of the drug in a hospital from May 2017 to June 2018 in the process of clinical ward round and medical examination. Results 198 cases of irrational medication were intervened by clinical pharmacists with most of which occurred on Monday( 33. 84%),the irrational drug type was not suitable for use( 63. 13%), and the unreasonable drug type was mostly antibacterial( 52. 58%). Conclusion Clinical pharmacists' medication interven- tion plays an important role in ensuring the safe and effective use of drugs,which is conducive to promoting rational drug use and improving the quality of clinical pharmacists.
