糖皮质激素对AECOPD并肾上腺功能不全患者呼吸机撤离结局的影响

Effect of glucocorticoid on the outcome of ventilator evacuation in AECOPD patients with adrenal insufficiency

目的分析糖皮质激素对AECOPD并肾上腺皮质功能不全(AI)患者呼吸机撤离结局的影响.方法选取医院ICU 2014年10月-2016年4月收治的AECOPD并AI患者80例,随机分为观察组和对照组各40例,对照组采用布地奈德治疗,观察组采用糖皮质激素治疗.比较2组呼吸机撤离成功率、肺部感染控制时间、医疗费用及ICU住院时间.结果观察组患者的呼吸机撤离成功率为95.00%高于对照组的75.00%,肺部感染控制时间、ICU住院时间均短于对照组,医疗费用低于对照组,差异均有统计学意义(P<0.05).结论采用糖皮质激素治疗AECOPD并AI患者能有效提高呼吸机撤离成功率.

Objective To investigate the effect of glucocorticoid on the evacuation outcome of patients with AECOPD and adrenal insufficiency( AI). Methods Eighty patients with AECOPD and AI admitted to the hospital ICU from October 2014 to April 2016 were randomly divided into observation group and control group,each of 40 cases. Control group was treated with budesonide and observation group was treated with glucocorticoids. The success rate of ventilator evacuation,lung infection control time,medical expenses and ICU hospitalization time were compared. Results The ventilator evacuation success rate of observation group was 95. 00% higher than 75. 00% of control group,and the lung infection control time and ICU hospitaliza- tion time were shorter than control group,the medical expenses were lower than the control group,and the difference was statis- tically significant( P < 0. 05). Conclusion The use of glucocorticoid in treatment of AECOPD patients with AI can effectively increase the success rate of ventilator evacuation.
