功能性垂体腺瘤手术治疗后激素水平变化与肿瘤体积变化的相关性研究 被引量:4

摘要 目的分析功能性垂体腺瘤手术治疗后激素水平和肿瘤体积的变化,并探讨二者的相关性。方法回顾性分析采用手术切除治疗的32例泌乳素瘤和33例生长激素腺瘤病人的临床资料,测定术前、术后1个月的激素水平和肿瘤体积并统计分析。结果泌乳素瘤和生长激素腺瘤病人术后1个月激素水平与术前相比均明显下降(P <0.05),术后1个月肿瘤体积与术前相比明显减小(P <0.05)。泌乳素瘤和生长激素腺瘤病人中,激素差百分比和肿瘤体积差百分比均呈正相关(r=0.980,P <0.05;r=0.973,P <0.05)。结论功能性垂体腺瘤手术治疗后激素水平变化可以反映肿瘤体积的变化程度,内分泌激素水平的测定可以作为影像学判断肿瘤残余程度的补充。 Objective To analyze the changes of hormone levels and tumor volume after surgery for functional pituitary adenomas,and explore the correlation between them. Methods Clinical data of 32 patients with prolactin adenomas and 33 patients with growth hormone(GH) adenomas, who received surgical resection of the tumor, were analyzed retrospectively. The hormone levels and tumor volumes before and 1 month after surgery were measured and analyzed statistically. Results Compared with the condition before surgery, the hormone levels decreased and the tumor volume shrank significantly 1 month after surgery in prolactin adenoma or GH adenoma patients(P < 0.05). The percentage of hormone difference was positively correlated with the percentage of tumor-volume difference in prolactin adenoma or GH adenoma patients(r = 0.980, P < 0.05;r = 0.973, P < 0.05). Conclusions The changes of hormone levels after surgery of functional pituitary adenoma can reflect the changes of tumor volumes. The determination of endocrine hormone level can be used as a supplement to imaging for judging the amount of residual tumor.
