晚期肿瘤患者家属选择临终决定的影响因素调查分析

Investigation and Analysis of the Influencing Factors for the Family Members of Patients With Advanced Cancer to Choose End-of-life Decision

目的了解晚期肿瘤患者家属对临终决定的真实看法及其影响因素。方法收集51例晚期肿瘤患者家属的问卷资料,进行分析。结果显示家属文化程度、家庭经济状况、临终决定的决策权不明确、对生命的尊重和死亡观的改变是晚期肿瘤患者选择临终决定的影响因素。结论晚期肿瘤患者家属对临终决定的临床决策受到多种因素的影响,医疗保险制度需要进一步改进,以便更多患有晚期癌症的患者不会因为经济原因而放弃或过度治疗;逐步建立由患者、家属和医护人员共同组成的决策共享模式,根据个人情况告知患者,让更多患者及其家属积极参与临床决策过程,从而减轻家属因同意放弃治疗而产生的的巨大心理压力。

Objective To understand the real views of family members of patients with advanced cancer on end-of-life decisions and their influencing factors. Methods The questionnaire data of 51 family members of patients with advanced cancer were collected and analyzed. Results It showed that family members' educational level, family economic status, unclear decision-making right of end-of-life decision, respect for life and change of death view were the influencing factors for patients with advanced cancer to choose end-of-life decision. Conclusion Family members of patients with advanced cancer are influenced by many factors in the clinical decision-making of end-of-life decisions, and the medical insurance system should be further improved so that more patients with advanced cancer will not give up or over-treat due to economic reasons.
