Nursing Experience of One Case with Multiple Injuries Difficult to Off-line

目的总结1例多发伤患者困难脱机的护理经验。方法根据该患者的护理难点,在呼吸系统管理、原发疾病管理、管道管理、CRRT管理、营养支持等方面采取了一系列针对性护理措施。结果通过病因治疗,感染控制,不断调整脱机锻炼计划,加强气道护理、心理护理及营养支持,患者最终成功脱机。结论呼吸机应用中出现脱机困难的现象较高,找出引起脱机困难的病因,祛除病因及序贯脱机,在脱机锻炼过程中及时地进行感染控制、心肺功能评估、了解患者病情变化并及时处理,行之有效地进行脱机锻炼计划,加强营养和术后病情观察,可以极大提高脱机的成功率。

Objective To summarize the nursing experience of a patient with multiple injuries who had difficulty weaning. Methods According to the nursing difficulties of the patient, a series of targeted nursing measures were taken in respiratory system management, primary disease management, pipeline management, CRRT management and nutritional support. Results Through etiological treatment, infection control, continuous adjustment of off-line exercise plan, strengthening airway nursing, psychological nursing and nutritional support, patients finally successfully off-line. Conclusion The phenomenon of difficulty in weaning is relatively high in the application of ventilator. Finding out the causes of difficulty, eliminating the causes and sequential weaning, timely infection control, cardiopulmonary function assessment, understanding the patient’s condition changes and timely treatment in the process of off-line exercise, effective off-line exercise plan, strengthening nutrition and post-operative condition observation can greatly improve the success rate of weaning.
