超声乳化联合房角分离手术治疗白内障合并青光眼的应用效果及并发症发生率影响观察

Observation of Application Effect and Complications Incidence of Phacoemulsification combined with Goniosynechialysis on Cataract Complicated with Glaucoma

目的分析白内障合并青光眼患者行超声乳化联合房角分离手术治疗的效果,评定对并发症发生率的影响。方法对2016年5月至2017年5月前来就诊的白内障合并青光眼患者共54例进行数据收集,通过随机数字表法的形式分为对照组25例(25眼)以及实验组29例(29眼),对照组行常规手术救治,实验组行白内障超声乳化联合房角分离手术治疗,对比两组患者治疗后的临床总有效率以及并发症发生率。结果治疗前各项指标对比不存在差异性(P>0.05),治疗后实验组眼压以及视力指标分别为(17.7±5.6)mmHg,(0.61±0.21)mmHg,对照组分别为(24.3±8.8)mmHg,(0.42±0.22)mmHg,两组比较统计学意义存在。治疗前,前后房角宽度以及中央前房深度水平不具有统计学意义(P>0.05),治疗后实验组优于对照组,组间比较差异性显著(P<0.05)。并发症发生率分析,对照组前房出血3例、浅前房2例、角膜水肿3例、无菌性反应2例,实验组分别为1例、2例、1例,组间比较差异性显著(P<0.05)。结论白内障超声乳化联合房角分离手术治疗白内障并发青光眼的效果显著,降低并发症发生率,改善眼压指标以及视力。

Objective To analyze curative effect of phacoemulsification combined with goniosynechialysis on cataract patients with glaucoma,evaluate effect on complications incidence.Methods Collect data of 54 cases cataract and glaucoma patients from May 2016 to May 2017.Divide them into control group (25 eyes) and experimental group (29 eyes) with random digital table method.Control group was treated with routine surgery,and xperimental group with phacoemulsification combined with goniosynechialysis.Compare clinical effects,total effective rate and complications incidence of two groups after treatment.Results indexes showed no difference before treatment (P>0.05).After treatment,intraocular pressure and visual acuity of experimental group was (17.7±5.6) mmHg,(0.61±0.21) mmHg, and that of te control group was (24.3±8.8) mmHg,(0.42±0.22) mmHg respectively,comparison showed statistical significance between two groups.Anterior and posterior chamber angle width and central anterior chamber depth showed no statistical significance before and after treatment (P>0.05).After treatment,that of experimental group was better than control group,difference between two groups was significant (P<0.05).For complications incidence,control group has 3 cases of anterior chamber hemorrhage,2 cases of shallow anterior chamber,3 cases of corneal edema and 2 cases of aseptic reactionm,there were 1 case,2 cases and 1 case respectively in experimental group,difference showed significance between two groups (P<0.05).Conclusion phacoemulsification combined with goniosynechialysis has significant effect in treatment of cataract complicated with glaucoma,which can reduce complications incidence, improve intraocular pressure indexes and visual acuity.
