hs-TNⅠ和CK-MBmass检测在急性心肌梗死诊断中的应用价值 被引量:5

Application value of hs-TNⅠand CK-MBmass in diagnosis of acute myocardial infarction

摘要 目的探讨超敏肌钙蛋白I(high-sensitivity troponinⅠ,hs-TNⅠ)和肌酸激酶同工酶质量(creatine kinase-MB mass,CK-MBmass)联合检测对急性心肌梗死(acute myocardial infarction,AMI)的应用价值。方法采用化学发光法,对哈尔滨医科大学附属第一医院73例AMI患者和110例健康体检者hs-TNⅠ水平和CK-MBmass进行检测。结果AMI组hs-TnI、CK-MBmass各阶段水平及阳性率均明显高于正常对照组(P<0.05);AMI组hs-TNⅠ各阶段阳性率(93%、97%、100%、78%)与CK-MBmass阳性率(25%、56%、84%、11%)相比较,hs-TNⅠ的敏感性和特异性高于CK-MBmass,但在2-3 d时hs-TNⅠ具有78%的阳性率,诊断窗口期长,而CK-MBmass 2-3d时阳性率已降为11%,大多说明治疗有效。结论hs-TNI和CK-MBmass联合检测对AMI的诊断、治疗和预后判断有重要意义。 Objective To explore the application value of combined detection of high-sensitivity troponinⅠ(hs-TNⅠ)and creatine kinase-MB mass(CK-MBmass)for acute myocardial infarction(AMI).Methods The levels of hs-TNⅠand CK-MBmass were determined by chemiluminescence in 110 healthy patients and 73 patients with AMI.Results The levels and positive rate of hs-TNⅠand CK-MBmass in patients with AMI were significantly higher than those in the normal control group(P<0.05).The positive rates of hs-TNⅠat all stages of patients whith AMI were 93%,97%,100%and 78%,while the positive rate of ck-mbmass was 25%,56%,84%and 11%,the sensitivity and specificity of hs-TNⅠwere higher than CK-MBmass.As well as,the positive rate of hs-TNⅠin 2~3 days had a long window period,while the positive rate of CK-MBmass at 2-3 days had been reduced to 11%,which indicatied that most treatments were effective.Conclusion The combined detection of hs-TNⅠand CK-MBmass is important to guide clinical therapy and judge prognostic of AMI.
