无鼻内镜下RS置管术治疗泪道阻塞的临床疗效

Clinical outcomes of RS lacrimal tube on lacrimal duct obstruction without nasal endoscope

目的探讨无鼻内镜下RS型泪道再通管(RS管)治疗泪道阻塞的临床疗效。方法邯郸市眼科医院2016年8月—2017年12月收治的62例80只眼泪道阻塞患者的临床资料。根据入院时间分为2组,治疗组36例47只眼选用RS管,对照组26例33眼选用环形泪道引流管,对比分析2组患者的术中鼻出血、临床疗效和术后并发症的情况。结果治疗组术中无鼻出血发生,对照组鼻黏膜出血33例,2组比较差异有统计学意义(P<0.05)。治疗组治愈41例,好转4例,无效2例,有效率95.7%;对照组治愈21例,好转8例,无效4例,有效率87.9%,2组比较有统计学意义(P<0.05)。治疗组中脱管1例,脱管率为2.1%;对照组中脱管2例,脱管率为6.1%,脱管原因均为患者操作不当,给予重新置管,随访时泪道冲洗通畅。2组脱管率比较,无统计学意义(P>0.05)。结论RS管植入术治疗泪道阻塞手术效果好、操作简便、时间短、无需勾管、拔管、并发症少、安全性高。

OBJECTIVE To explore the clinical outcomes of RS lacrimal tube on lacrimal duct obstruction without nasal endoscope. METHODS A retrospective analysis from August 2016 to December 2017 had been done. Sixty-two patients (80 eyes) with lacrimal duct obstruction was divided into two groups according to the time of admission. Treatment group consisted of 36 cases (47 eyes) while control group included 26 cases (33 eyes). The treatment results were compared and analyzed. RESULTS No eye suffered nasal mucosa hemorrhage in treatment group while 33 eyes encountered this condition in control group,the difference between the two groups was statistically significant (P<0.05). In treatment group,41 eyes were cured,4 eyes were improved,2 eyes showed no effect and effective rate was 95.7%. In control group,21 eyes were cured,8 eyes were improved,4 eyes indicated no effect and effective rate was 87.9%,the difference between the two groups was significant (P<0.05). Postoperative tube detachment rate was 2.1% in the treatment group and 6.1% in the control group. No statistical significance was found in the two groups (P>0.05). CONCLUSIONS The RS lacrimal tube was easy and quick to operate and with favorable clinical effect,less adverse effects,high safety and free of tube adjustment or removement in treatment of lacrimal duct obstruction.
