弥可保联合鼠神经生长因子治疗带状疱疹神经痛的疗效及IL-6、IL-10的变化

Effect of mecobalamin plus mouse nerve growth factor on neuralgia and changes of IL-6 and IL-10 of patients with herpes zoster

目的观察弥可保联合鼠神经生长因子治疗带状疱疹患者神经痛的疗效及IL-6、IL-10水平的变化。方法选取60例带状疱疹患者,根据患者自愿选择治疗方式分为观察组及对照组,两组患者均给予更昔洛韦抗病毒及弥可保营养神经治疗,观察组在对照组用药基础上加用鼠神经生长因子。对比两组患者治疗前、治疗后第1周、第2周及第4周神经痛觉模拟评分(VAS)和睡眠质量评分(QS),评价临床疗效。比较两组患者带状疱疹后遗神经痛(PHN)的发生率及白细胞介素-6(IL-6)、白细胞介素-10(IL-10)水平的变化。结果与同组治疗前比较,治疗后患者VAS评分明显下降(P<0.05)、QS评分显著上升(P<0.05)。观察组与对照组VAS及QS评分分别于第2周和第4周比较,差异显著(P<0.05)。第4周与同组第2周VAS及QS评分比较,差异有统计学意义(P<0.05)。观察组PHN发生率低于对照组。治疗两周后两组患者IL-6及IL-10水平均下降,与治疗前比较,差异明显(P<0.05),观察组IL-6水平较对照组明显下降(P<0.05)。结论弥可保联合鼠神经生长因子的治疗可有效缓解带状疱疹所致神经痛程度,降低PHN的发生,可供同行借鉴。

Objective To observe the efficacy of mecobalamin plus mouse nerve growth factor on neuralgia and changes of IL-6 and IL-10 of patients with herpes zoster. Methods Sixty patients with herpes zoster were collected and divided into two groups. Two groups of patients were given therapy of ganciclovir and Mecobalamin,while the observation group was given mouse nerve growth factor. Vasual analogue score ( VAS),sleep quality score ( QS) were used to assess the clinical outcomes. The incidence of postherpetic neuralgia ( PHN) and the changes of interleukin-6 ( IL-6) and interleukin-10 ( IL-10) levels were compared after treatment. Results After treatment,VAS score showed a downward trend,QS score showed an upward trend. Compared with before treatment,there were significant difference. The difference between VAS and QS scores was statistically significant at the fourth and the second week in the same group ( P < 0. 05). The VAS and QS scores in the second and fourth weeks were signifi- cantly different between the observation group and the control group ( P < 0. 05). The inci- dence of PHN in the observation group was lower than that in the control group. The levels of IL-6 and IL-10 in the two groups decreased after treatment,and there was significant difference between the two groups ( P < 0. 05). The level of IL-6 decreased in the observation group after two weeks treatment ( P < 0. 05 ). Conclusion Mecobalamin combined with mouse nerve growth factor in treatment of herpes zoster is effectively and could reduce the incidence of PHN. It is worthy of clinical promotion.
