艾滋病病毒感染合并慢性乙型肝炎的抗病毒治疗方案选择与疗效分析

Antiviral treatment options and effect analysis of HIV infection with chronic hepatitis B

目的探讨艾滋病病毒感染合并慢性乙型肝炎的抗病毒治疗方案选择与疗效分析。方法选择2015年1月~2018年9月间由哈尔滨医科大学附属第四医院收治的艾滋病病毒感染合并慢性乙型肝炎患者100例为本次研究对象,随机将患者分为2组,分别为观察组和对照组,每组50例;观察组患者应用替诺福韦酯,拉米夫定,依非韦伦联合治疗,对照组患者应用齐多夫定,拉米夫定,依非韦伦联合治疗,比较分析两组患者的临床治疗效果。结果治疗前两组患者的肝功能与CD4^+ T细胞数相比无明显差异(P>0.05),治疗后观察组患者的肝功能各项指标及CD4^+ T细胞数均优于对照组,两组相比差异有统计学意义(P<0.05),观察组患者HIV-RNA和HBV-DNA转阴率显著高于对照组,两组相比差异有统计学意义(P<0.05)。结论含有替诺福韦酯的治疗方案可以有效抑制HIV-RNA与HBV-DNA的复制,使CD4^+ T细胞数显著增加,有效改善患者的肝功能状况。

Objective To explore the selection and effect of antiviral therapy for HIV infection combined with chronic hepatitis B. Methods One hundred patients infection with HIV/HBV were selected from January 2015 to September 2018 in the Fourth Affiliated Hospital of Harbin Medical University, and randomly divided into two groups: observation group and control group, 50 cases in each group. Observation group were treated with Tenofovir, Lamivudine and Efavirenz Tablets, and control group were treated with Zidovudine, Lamivudine, and Efavirenz Tablets. The clinical treatment effect of the two groups was compared and analyzed. Results There was no significant difference in liver function and CD4^+ T cells number before treatment(P>0.05). The liver function and CD4^+ T cells number of the observation group were better than those of the control group after treatment(P<0.05). The HIV-RNA and HBV-DNA negative rate in the observation group was significantly higher than that in the control group(P<0.05). Conclusion The treatment regimen containing tenofovir can effectively inhibit the replication of HIV-RNA and HBV-DNA, significantly increase the CD4^+ T cells number and effectively improve the liver function.
