重度烧伤患者中药浸浴后应用中西医联合换药的疗效及对炎症指标的影响

Effect of Traditional Chinese and Western Medicine Combination Dressing Change on Inflammatory Indexes in Patients with Severe Burn after Immersion in Traditional Chinese Medicine

目的:观察重度烧伤患者中药浸浴后应用中西医联合换药的治疗效果及对炎症指标的影响。方法:选取2010年1月~2017年12月我院收治的117例重度烧伤患者,采用随机分配的方法分为两组,对照组和研究组,对照组(n=58)先采用洗必泰溶液对创面进行清洗然后在将磺胺嘧啶银霜对创面进行涂抹,研究组(n=59)采用中药浸浴然后实施中西医联合换药进行治疗,对两组患者临床治疗效果、患者在换药时的疼痛情况、患者7天内血清炎症因子IL-6、TNF-α指标以及创面愈合时间和瘢痕发生情况进行比较。结果:两组患者实施不同方法治疗后研究组患者的临床治疗有效率明显高于对照组(P<0.05),研究组患者换药时的疼痛情况显著低于对照组(P<0.05),研究组患者治疗后7天内血清炎症因子IL-6、TNF-α指标均低于对照组(P<0.05),研究组患者创面愈合时间和瘢痕发生情况明显优于对照组(P<0.05)。结论:对重度烧伤患者采用中药浸浴然后实施中西医联合换药进行治疗临床治疗效果较为显著,可以有效的控制患者创面的炎症因子指标,值得在临床上进行广泛推广使用。

Objective: To explore the therapeutic effect of traditional Chinese medicine combined with western medicine for severe burn patients and its effect on inflammatory indexes. Methods: 117severe burn patients treated in our hospital from January 2010to December 2017were divided into two groups which was controlled group and study group. The controlled group ( n = 58) first cleaned the wound with Chlorhexidine Solution and then applied the Sulfadiazine Silver Cream to the wound. The study group ( n = 59) used traditional Chinese medicine. The clinical treatment effect of the two groups of patients,the pain of the patients during the change,the serum inflammatory factor IL-6,TNF-alpha,the wound healing time and the scar were compared between the two groups. Results: The effective rate of clinical treatment in the two groups was significantly higher than that of the controlled group ( P<0. 05). The pain in the study group was significantly lower than that of the controlled group ( P<0. 05). The serum inflammatory factors IL-6and TNF-alpha in the study group were lower than the controlled group ( P<0. 05) within 7days after the treatment. The wound healing time and scar formation in the study group were significantly better than those in the controlled group ( P<0. 05). Conclusion: The curative effect of severe burn patients after immersion in traditional Chinese medicine treated with traditional Chinese medicine and western medicine combination dressing change is more effective. It can effectively control the index of inflammatory factors in the wound. It is worthy of extensive use in clinical practice.
