甲硝唑联合乳酸菌制剂治疗妊娠期滴虫性阴道炎的Meta分析

The Clinical Efficacy and Safety of Metronidazole Combined with Lactobacillus in Treatment of Trichomonas Vaginitis During Pregnancy: a Meta Analysis

目的通过Meta分析探讨口服甲硝唑联合乳酸菌制剂阴道给药治疗妊娠期滴虫性阴道炎的疗效及安全性。方法计算机检索PubMed数据库、Cochrane图书馆、CNKI、万方数据库及维普数据库,并进一步对纳入文献的参考文献进行扩大检索。对符合纳入标准的随机对照研究,采用RevMen5.3软件进行Meta分析。结果共纳入研究23篇,患者2323例。甲硝唑与乳酸菌制剂联用治疗妊娠期滴虫性阴道炎的总有效率优于单用甲硝[p[0.001,RR=1.22,95%CI(1.18,1.26)],治愈率高[p[0.001,RR=1.42,95%CI(1.31,1.54)],复发率低[p[0.001,RR=0.20,95%CI(0.13,0.31)],不良反应发生率低[p[0.001,RR=0.42,95%CI(0.24,0.73)],差异均有统计学意义。结论Meta分析结果表明甲硝唑与乳酸菌制剂联用治疗妊娠期滴虫性阴道炎疗效优于单用甲硝唑,复发率低,不良反应发生率低,安全性高,值得临床推广使用。

Objective To analyse the clinical efficacy and safety of oral metronidazole combined with lactobacillus vaginal administration (experimental group) and oral fluconazole alone (control group) in the treatment of trichomonas vaginitis during pregnancy by Meta analysis. Methods The randomized controlled trials (RCT) were retrieved from PubMed, Cochrane library, CNKI, Wanfang database and Weip database and references listed in all studies.Meta-analysis was conducted using RevMen 5.3 software. Results A total of 23 studies and 2323 patients were included. Oral metronidazole combined with lactobacillus vaginal administration had higher overall efficiency [p<0.001, RR=1.22, 95%CI (1.18,1.26)], cure rate were higher[p<0.001, RR=1.42, 95%CI (1.31,1.54)], recurrence rate was lower[p<0.001, RR=0.20, 95%CI (0.13,0.31)]and adverse reaction ratewas lower[p<0.001, RR=0.42, 95%CI (0.24,0.73)], the difference was statistically significant. Conclusion Meta analysis showed that oral metronidazole combined with lactobacillus vaginal administration had higher overall efficiency, the effect in the combined groups is better, and there currence rate is lower.
