摘要
目的研究特拉唑嗪联合体外冲击波治疗ⅢB型前列腺炎的疗效及对患者血清IL-8、TNF-α、IL-1β的影响.方法选取本院2017年4月至2018年3月间收治的ⅢB型前列腺炎患者82例,根据不同的治疗方式将其平均分为观察组(41例)与对照组(41例).对照组给予特拉唑嗪治疗,观察组给予特拉唑嗪联合体外冲击波治疗,比较两组患者的疗效以及对患者血清IL-8、TNF-α、IL-1β表达的影响.结果观察组的治疗总有效率[90.24%(37/41)]显著高于对照组[65.85%(27/41)](χ^2=7.02,P=0.01);治疗后,观察组的NIH-CPSI和IPSS评分[(13.21±1.35)、(8.10±1.66)]均显著低于对照组[(18.90±1.23)、(13.25±1.78),P<0.05],前列腺液和尿液中的TNF-α、IL-1β、IL-8表达水平均显著低于对照组(P<0.05),前列腺液中的卵磷脂小体表达显著高于对照组(P<0.05).结论特拉唑嗪联合体外冲击波可有效缓解ⅢB型前列腺炎患者临床症状,抑制患者前列腺炎症反应,提高免疫功能及临床疗效,疗效显著.
Objective To investigate the therapeutic effect of terazosin combined with extra-corporeal shock wave therapy on type IIIB prostatitis and its influence on serum IL-8, TNF-α and IL-1β. Methods From April 2017 to March 2018, 82 cases of type IIIB prostatitis treated in our hospi-tal were selected. According to different treatment methods, they were evenly divided into study group (41 cases) and control group(41 cases). The control group was treated with terazosin, and the study group was treated with terazosin combined with extracorporeal shock wave therapy. The curative effects of the two groups were compared, and the terazosin combined with extracorporeal shock wave therapy on serum IL-8, TNF-α, IL-1βexpression in patients. Results The total effective rate in the observa-tion group (90. 24%) was significantly higher than that in the control group (65. 85%)(χ2 =7. 02, P=0. 01). After treatment, the NIH -CPSI and IPSS scores of the observation group [(13. 21 ± 1. 35),(8. 10 ± 1. 66)] were significantly lower than the control group [(18. 90 ± 1. 23),(13. 25 ± 1. 78)(P<0. 05)]. The expression levels of TNF-α, IL-1βand IL-8 in prostatic fluid and urine were significantly lower than those in the control group (P<0. 05), the expression of lecithin in the prostat-ic fluid was significantly higher than that in the control group ( P<0. 05 ). Conclusions Terazosin combined with extracorporeal shock wave can effectively relieve the clinical symptoms of patients with type IIIB prostatitis, significantly inhibit the local inflammatory reaction of the prostate, and has a sig-nificant effect.
