探讨Devine术不同固定位置治疗真性隐匿性阴茎的疗效

Efficacy of different fixed position of Devine in the treatment of true concealed penis

目的探讨Devine术不同固定位置治疗真性隐匿性阴茎的疗效.方法回顾性分析2014年3月至2016年2月在本院行Devine术的真性隐匿性阴茎患者40例的临床资料.按照患者术中固定方法分为两组,其中观察组(30例)将阴茎根部与阴茎背侧皮下固定,对照组(10例)将阴茎体根部与阴茎体腹侧皮下固定.观察比较两组患者手术时间、术中出血率、水肿时间,术前、术后不同时间段阴茎体显露长度,术后视觉模拟评分(VAS评分)、阴茎回缩率以及并发症发生情况.结果两组患者手术出血量比较差异无统计学意义(P>0.05),观察组患者治疗后手术时间、水肿时间明显短于对照组,两者对比差异具有统计学意义(P<0.05).治疗前、治疗后1周、治疗后4周,两组患者阴茎体显露长度比较差异无统计学意义(P>0.05),观察组患者治疗后12周、24周阴茎体显露长度均明显大于对照组,对比差异具有统计学意义(P<0.05).两组患者治疗后阴茎回缩率对比差异无统计学意义(P>0.05),观察组患者治疗后VAS评分明显低于对照组,两者比较差异具有统计学意义(P<0.05).结论在使用Devine术治疗真性隐匿性阴茎时,将阴茎根部与阴茎背侧皮下固定可显著缩短手术和水肿时间,患者疼痛程度低,阴茎显露长度改善效果显著,阴茎体显露更好,值得临床推广应用.

Objective To investigate the effect of different fixed position of Devine in the treatment of true concealed penis. Methods From March 2014 to February 2016, 40 patients with true concealed penis who underwent Devine operation were selected retrospectively. The patients were divided into two groups according to the postoperative fixation methods, of which the observation group (30 cases) fixed the penis root and the dorsal side of the penis subcutaneously, and the control group (10 cases) fixed the penis root and the penis ventral side subcutaneously. The operation time of the two groups, the rate of bleeding, the time of edema, the length of the exposure of the penis before and after the operation, the postoperative visual analogue score ( VAS score), the penile retraction rate and the occurrence of the complications were observed. Results There was no difference in the amount of bleeding between the two groups(P>0. 05). The time of operation and the time of edema in the obser-vation group were significantly shorter than those in the control group, and the difference between the two groups was statistically significant(P<0. 05). Before treatment, 1 week treatment and 4 weeks af-ter treatment, there was no difference in the length of the penis body exposure in the two groups( P>0. 05). The exposure length of the penis body in the observation group was significantly greater than that of the control group at 12 and 24 weeks after treatment, and the difference was statistically signifi-cant( P<0. 05 ). There was no difference between the two groups of patients after treatment ( P >0. 05). The VAS score of the patients in the observation group was significantly lower than that of the control group. The difference between the two groups was statistically significant(P<0. 05). Conclu-sions When Devine is used to treat the true concealed penis, the operation and edema time of the penis and the dorsal side of the penis can be shortened significantly, the degree of pain in the patients is low, the length of the penis is improved significantly and the penis body is exposed better, and it is worthy of clinical application.
