摘要
阐述出院计划的定义、意义、流程、实施内容等,以及近年来国内外出院计划的研究现状及在慢性心力衰竭病人中应用情况,分析目前我国出院计划实施的不足,从而促进出院计划在我国慢性心力衰竭病人中的实施。
This article explained the definition,significance,process,and implementation content of the discharge plan,as well as the research status quo of the discharge plan in China and abroad in recent years.It reviewed the application status in patients with chronic heart failure and analyzed the current implementation disadvantage of the discharge plan in China,thus promoting the discharge plan implementation in patients with chronic heart failure in China.
