腮腺嗜酸细胞腺瘤的CT表现分析

目的分析腮腺嗜酸细胞腺瘤的CT表现,以提高对该少见疾病的认识。方法回顾性分析经手术病理证实的12例腮腺嗜酸细胞腺瘤的CT资料。12例中男7例,女5例,中位年龄56岁(31~73岁)。对肿瘤的大小、边界、数目、形态、密度、强化方式、周围侵犯、淋巴结转移等进行分析。结果12例患者均为单发肿瘤,右侧7个,左侧5个;9例位于腮腺浅叶,2例位于腮腺深叶,1例跨深、浅叶;肿瘤最大径1.1~2.9cm,平均(2.1±0.3)cm;病灶呈类圆形8例,椭圆形2例,不规则分叶状2例;边缘光整10例,不光整2例,边界均清晰,未见侵犯周围组织;CT平扫病灶呈等密度8例,低密度4例,未见明显钙化;病灶均呈实性,密度均匀9例,不均匀3例;增强后病灶呈明显强化10例,中度强化2例。结论单发、边缘光整、少见钙化囊变、血供丰富的腮腺浅叶实性占位灶,有助于提示PGO的诊断。

Objective To explore the CT imaging findings of parotid gland oncocytoma in order to improve the understanding of this rare disease.Methods The CT imaging characteristics of 12 patients(7 males and 5 females,median age 56 years old)with pathologically proved PGO were retrospectively reviewed.CT images were evaluated in relation to the following:location,size,margin,contour,pattern of enhancement,invasion of surrounding structures and lymph node metastasis.Results Twelve cases of PGO have single tumor,seven lesions on the right and five on the left.Nine lesions were located in the superficial lobe,2 in the deep 1 obe and 1 across the superficial and deep 1 obe.The maximum diameter ranged from 1.1~2.9 cm with mean of(2.1±0.3)cm.Eight cases demonstrated round masses,two cases demonstrated oval round masses and two cases demonstrated lobular masses.The boundary were well-defined in 10 cases and ill-defined in 2 cases.All the 12 lesions were well circumscribed with smooth contours,none of 12 cases invaded the adjacent tissue.8 lesions manifested as iso-density and 4 lesions manifested as low-density on non-enhanced CT,None of 12 lesions showed calcification.All lesions displayed solid appearances,the masses were homogeneous in 9 lesions and hoterogeneous in 3 lesions.On post contrast images,Lesions demonstrated remarkable enhancement in 10 cases,moderate enhancement in 2 cases.Conclusion Solitary,well-defined masss in the superficial lobe of parotid gland accompanied with rare calcification,cystic degeneration or necrosis and rich blood supply can suggest the diagnosis of PGO,CT examinations can accurately demonstrated the extent of tumor involvement.
