摘要
目的通过MSCT血管成像分析先天性心脏病合并支气管异常诊断价值。方法选择我院2013年5月~2015年5月间经手术治疗的35例婴幼儿(6月~7岁)复杂性先天性心脏病患者作为研究对象,通过VRT、MPR、MIP和MinP等后处理技术,评估患儿心脏血管畸形情况和气管支气管异常情况。结果MSCT显示共存在心血管畸形104处,诊断准确率分别为97.2%。其中心内畸形诊断准确率分别为98.2%(57/58);心脏-大血管连接异常诊断准确率为100.0%(24/24);心外大血管畸形诊断准确率为92.0%(23/25)。35例患儿共检出气管性支气管12例(12/35),Ⅰ型3例,Ⅱ型2例,Ⅲ型5例,Ⅳ型2例。检出气管狭窄6例(6/35),气管憩室1例。结论先天性心脏病患儿气管支气管异常发生率较高,MSCT血管成像在评估心脏血管异常的基础上可以准确评估气管支气管异常情况。
ObjectiveTo analyze congenital heart disease with bronchial abnormalities by multislice spiral CT angiography.MethodsWe selected 35 infants and children who had undergone surgery in our hospital from May 2013 to May 2015(June-7-year-old)patients with complex congenital heart disease as the study object.Through VRT,MPR and MIP and other post-processing techniques,we assessed cardiac vascular malformations and tracheobronchial abnormalities in children.ResultsMSCT showed a total of 104 cardiovascular malformations with a diagnostic accuracy of 97.2%.The diagnostic accuracy of the malformation in the center was 98.2%(57/58),the accuracy of diagnosis of cardiac-macrovascular connection abnormality was 100.0%(24/24),and the diagnostic accuracy of extracardiac vascular malformation was 92.0%(23/25).Thirteen patients(12/35)with tracheal bronchus were detected in 35 patients,including 3 cases of typeⅠ,2 cases of typeⅡ,5 cases of typeⅢ,and 2 cases of type IV.Six cases(6/35)of tracheal stenosis were detected,and one case of tracheal diverticulum.ConclusionThe incidence of tracheobronchial abnormalities in children with congenital heart disease is high.Multi-slice spiral CT angiography can accurately assess the abnormalities of tracheobronchial on the basis of assessment of cardiac vascular abnormalities.
