摘要
目的探讨三维超声Omniview联合VCI(Omniview+VCI)图像后处理技术在黏膜下子宫肌瘤分型中的临床应用价值。方法按照欧洲宫腔镜学会黏膜下肌瘤的分类标准对肌瘤进行分型,对63例黏膜下子宫肌瘤患者的二维(2D-TVS)、三维(Omniview+VCI)超声图像进行回顾性分析,并与宫腔镜手术结果比较。结果与宫腔镜手术分型比较,2D-TVS共确诊47例,总体敏感度为74.60%。Omniview+VCI共确诊58例,总体敏感度为92.06%。两者比较有统计学差异(P<0.01)。对0型及I型的诊断敏感度两种检查方法没有明显差异(P=0.281,P=0.475)。对II型黏膜下肌瘤,两种诊断方法比较差异有统计学意义(P=0.038)。结论三维超声Omniview联合VCI成像技术对黏膜下子宫肌瘤的分型诊断具有较高的诊断正确率,尤其对II型诊断具有明显优势,具有较好的临床应用价值。
Objective To evaluate the clinical value of transvaginal three-dimensional free anatomic section imaging technique(Omniview)combined with volume contrast imaging technique(VCI)in the diagnosis of classification of submucous fibroids.Methods The fibroids were classified according to the European Society of Hysteroscopy Classification of Submucous Fibroids.A retrospective analysis of two-dimensional(2 D-TVS)and three-dimensional(Omniview+VCI)ultrasound images was performed in 63 patients with submucosal myoma of uterus compared with pathological results of surgery.Results Compared with hysteroscopic surgery classification,a total of 47 cases were diagnosed with 2 D-TVS,and the overall sensitivity was 74.60%.Omniview+VCI diagnosed 58 cases,with an overall sensitivity of 92.06%.There were statistically significant differences between the two(P<0.01).There was no significant difference in the diagnostic sensitivity of type 0 and type I(P=0.281,P=0.475).For type II submucosal fibroids,the difference between the two diagnostic methods was statistically significant(P=0.038).Conclusion Three-dimensional Omniview combined with VCI imaging technology has a high diagnostic accuracy rate for submucosal fibroid diagnosis,especially for type II diagnosis,which has a good clinical application value.
