MicroRNA-21在胰腺癌患者血清中的表达及临床意义

Expression and clinical significance of serum microRNA-21 in pancreatic cancer

目的探讨血清microRNA-21在胰腺癌患者及正常人群中的表达差异,分析血清microRNA-21的表达与胰腺癌患者临床病理特征的关系。方法前瞻性入组2015年1月至2018年12月首都医科大学附属北京友谊医院收治的60例胰腺癌患者及60例年龄性别匹配的正常人群,用microRNA提取试剂盒提取血清RNA,用实时荧光定量聚合酶链反应的方法检测microRNA-21的表达,并分析microRNA-21的表达与胰腺癌患者临床病理特征的关系及microRNA-21相对表达量对胰腺癌的诊断效能。结果胰腺癌患者血清microRNA-21相对表达量(3.75±1.81)明显高于正常人群(2.22±1.88),差异具有统计学意义(t=4.542,P<0.001),microRNA-21的表达与患者性别、年龄无关(t=0.396,t=-0.061;P>0.05),但与胰腺癌的分化程度、T分期、是否存在淋巴结转移和远处转移明显相关(t=3.633,t=-2.694,t=2.534,t=2.969;P=0.001,P=0.009,P=0.014,P=0.004)。以血清microRNA-21的相对表达量2.55为临界值,其诊断胰腺癌的敏感度和特异度分别为68%和75%。结论MicroRNA-21在胰腺癌患者血清中的表达明显高于正常人群,血清microRNA-21与胰腺癌分化程度、T分期、有无淋巴结转移和远处转移密切相关,检测血清microRNA-21的表达情况可能作为胰腺癌诊断的一项新的标志物。

Objective To investigate the expression of serum microRNA-21 in patients with pancreatic cancer and in healthy individuals,and to analyze the association of the expression with clinical and pathological characteristics in patients with pancreatic cancer.Methods In this prospective study,60 cases of pancreatic cancer patients and 60 age-and sex-matched healthy individuals from Beijing Friendship Hospital,Capital Medical University were enrolled from January 2015 to December 2018.Serum RNA was extracted by microRNA Isolation Kit.Real-time polymerase chain reaction(RT-PCR)was used to analyze microRNA-21 expression.The relationship between the expression of microRNA-21 and the clinicopathological characteristics of pancreatic cancer patients was analyzed,and the diagnostic efficacy of the relative expression of microRNA-21 in pancreatic cancer was evaluated.Results The relative expression of serum microRNA-21 was statistically higher in patients with pancreatic cancer than in healthy individuals[(3.75±1.81)vs.(2.22±1.88),t=4.542,P<0.001)].For patients with pancreatic cancer,the relative expression of microRNA-21 was closely associated with differentiation,tumor stage and lymph node metastasis,and distant metastasis(t=3.633,t=-2.694,t=2.534,t=2.969,respectively;P=0.001,P=0.009,P=0.014,P=0.004,respectively),but no statistical difference was observed between the relative expression of microRNA-21 and age or sex(t=0.396,t=-0.061,respectively,P>0.05).With the cut-off value of 2.55,the sensitivity and specificity of serum microRNA-21 relative expression for diagnosis of pancreatic cancer was 68%and 75%,respectively.Conclusion The expression of serum microRNA-21 was higher in patients with pancreatic cancer than in healthy individuals.For patients with pancreatic cancer,the high level of microRNA-21 was statistically associated with poor differentiation,advanced T stage,and the presence of lymph node metastasis and distant metastasis.The detection of microRNA-21 in serum might serve as a new tumor biomarker in the diagnosis of pancreatic cancer.
