真空辅助旋切术与经乳晕旁切口手术治疗乳腺良性肿物的疗效比较

Curative effect of vacuum assisted rotationis compared with that of periareolar incision in treatment of benign breast masses

目的比较临床应用真空辅助旋切术与经乳晕旁切口手术治疗乳腺良性肿物的疗效。方法采用前瞻性研究的方法,选取2016年5月至2018年3月在广西玉林市第一人民医院普通外科手术治疗82例乳腺良性肿物患者,按照随机平行分组法随机分为对照组和观察组,每组各41例,对照组患者给予经乳晕旁切口手术治疗,观察组患者给予真空辅助旋切术治疗,对比分析两组患者的手术时间、切口长度、术中出血量、手术费用、术后不良反应、半年复发率及乳房美观程度满意度情况。结果观察组患者的手术时间(16.61±3.23 min vs.28.42±1.75 min)、切口长度(2.32±0.43 mm vs.7.56±5.32 mm)、术中出血量(22.42±4.76 ml vs.34.36±3.91 ml)均明显低于对照组(P<0.05),手术费用(5 145.42±325.31元vs.3 217.25±167.70元)较对照组明显升高(P<0.05);两组患者的术后不良反应发生率比较,观察组明显低于对照组(17.1%vs.51.0%)(P<0.05),两组患者的半年内复发率比较(4.9%vs.4.9%),差异无统计学意义(P>0.05);两组患者对术后乳房美观程度满意度比较,观察组明显高于对照组(92.7%vs.82.9%)(P<0.05)。结论临床应用真空辅助旋切术治疗乳腺良性肿物疗效确切,不良反应发生率低,但手术费用相对较高。

Objective To compare the clinical efficacy of vacuum assisted rotection and periareolar incision in treatment of benign breast masses.Methods According to the prospective study,82 patients with benign breast tumors were treated with general surgery in the First People's Hospital of Yulin City,Guangxi during May 2016 to March 2018.They were randomly divided into control group and observation group according to randomized parallel grouping method.Forty-one patients in each group,patients in control group were treated with perivital incision,and patients in observation group were treated with vacuum-assisted resection.The operating time,length of incision,intraoperative blood loss,operation cost,and operation were compared between these two groups.Post-adverse reactions,half-year recurrence rate,and satisfaction with breast aesthetics were compared between these 2 groups.Results The operating time of patients in observation group(16.61±3.23 min vs.28.42±1.75 min),the length of incision(2.32±0.43 mm vs.7.56±5.32 mm),and the intraoperative blood loss(22.42±4.76 ml vs.34.36±3.91 ml)were significantly lower than those of control group.(P<0.05).The operation cost(5145.42±325.31 yuan vs.3 217.25±167.70 yuan)was significantly higher than that of control group(P<0.05).The incidence of postoperative adverse reactions in observation group was significantly lower than that of control group(17.1%vs.51.0%)(P<0.05).The recurrence rate in these two groups was(4.9%vs.4.9%),and the difference was not statistically significant(P>0.05).Patients in these two groups were satisfied with postoperative breast aesthetics.In comparison to the control group,the satisfaction of operation in patients of observation group was significantly higher than that of control group(92.7%vs.82.9%)(P<0.05).Conclusion The clinical application of vacuum assisted resection in treatment of benign breast masses is effective,and the incidence of adverse reactions is low,but the cost of surgery is relatively high.
