玻璃纤维桩氧化锆全瓷冠修复前牙牙体缺损效果及对患者咀嚼能力的影响

Effect of glass fiber post zirconia all-ceramic crown on restoration of anterior teeth defect and its influence on patients′masticatory ability

目的探讨玻璃纤维桩氧化锆全瓷冠修复前牙牙体缺损的效果及其对咀嚼能力的影响。方法选取2017年1~10月河北省唐山市协和医院口腔科收治的102例(128颗牙)前牙牙体缺损患者作为研究对象,采用随机数字表法将其分成观察组47例(60颗)和对照组55例(68颗);观察组给予玻璃纤维桩氧化锆全瓷冠修复,对照组给予金属桩钴铬烤瓷冠修复;观察两组修复成功率、修复效果、牙周、龈沟液、咀嚼效率和患者满意度。结果两组修复成功率比较,差异无统计学意义(P>0.05);观察组修复体完整度、颜色匹配、边缘适合性、边缘着色、牙龈状况合格人数均多于对照组,差异有统计学意义(P<0.05);修复后两组牙龈出血指数、牙周袋深度以及附着丧失水平较修复前显著提高(P<0.05),且观察组显著高于对照组(P<0.05);修复前两组龈沟液(GCF)和碱性磷酸酶(ALP)水平比较,差异无统计学意义(P>0.05);观察组修前复后GCF、ALP水平比较,差异无统计学意义(P>0.05),对照组修复前后GCF、ALP水平比较,差异有统计学意义(P<0.05),修复后观察组GCF、ALP水平均低于对照组(P<0.05);修复后1个月,两组健侧咀嚼效率比较,差异无统计学意义(P<0.05),观察组患侧咀嚼效率明显恢复,与同组健侧比较,差异无统计学意义(P>0.05),且明显高于对照组(P<0.05);观察组满意度明显高于对照组,差异有统计学意义(P<0.05)。结论玻璃纤维桩氧化锆全瓷冠修复前牙牙体缺损能够显著提高修复效果,对牙周组织和咀嚼能力影响小,患者满意度高。

Objective To investigate the effect of glass fiber post zirconia all-ceramic crown on the repair of anterior teeth defects and its effect on chewing ability.Methods A total of 102 patients(128 teeth)with anterior teeth defect admitted to Department of Stomatology,Tangshan Union Medical College Hospital in Hebei Province from January to October 2017 were selected.They were divided into observation group 47 cases(60 teeth)and control group 55 cases(68 teeth)by random number table method.The observation group was restored with glass fiber post zirconia all-ceramic crown,while the control group was restored with metal post cobalt-chromium ceramic crown.The success rate,restoration effect,periodontal,gingival crevicular fluid,masticatory efficiency and patient satisfaction of two groups were observed.Results There was no statistically significant difference in the repair success rate between the observation group and the control group(P>0.05).The number of eligible patients on restoration integrity,color matching,marginal fitness,marginal staining and gingival condition of the observation group were more than those of the control group,and the differences were statistically significant(P<0.05).After restoration,the gingival bleeding index,periodontal pocket depth,and attachment loss levels were significantly higher than those before restoration(P<0.05),and the observation group was significantly higher than the control group(P<0.05).There were no significant differences in the levels of gingival crevicular fluid(GCF)and alkaline phosphatase(ALP)between the two groups before restoration(P>0.05).There were no significant differences in the levels of GCF and ALP before and after repair in the observation group(P>0.05);there were significant differences in the levels of GCF and ALP before and after restoration in the control group(P<0.05).After restoration,the levels of GCF and ALP in the observation group were lower than those in the control group(P<0.05).One month after restoration,there was no difference in masticatory efficiency between the two groups(P<0.05).The masticatory efficiency of the affected side in the observation group recovered significantly,and there was no significant difference between the two groups(P>0.05),and it was significantly higher than that of the control group(P<0.05).The satisfaction rate of the observation group was higher than that of the control group(P<0.05).Conclusion Glass fiber post zirconia all-ceramic crown can significantly improve the restoration effect of anterior teeth defect,has little effect on periodontal tissue and masticatory ability,and has high satisfaction with patients.
