保留乳头乳晕乳腺癌改良根治术后即刻乳房再造对患者生存质量及远期预后的影响

Effects of immediate breast reconstruction after modified radical mastectomy with nipple-areola preservation on quality of life and long-term prognosis of patients

目的探讨保留乳头乳晕乳腺癌改良根治术后即刻乳房再造对患者生存质量及远期预后的影响。方法选取我院2006年3月至2016年3月收治的280例乳腺癌患者为研究对象,根据随机数字表法将其分为对照组和观察组,各140例。对照组采用保留乳头乳晕乳腺癌乳腺癌改良根治术治疗,观察组采用保留乳头乳晕乳腺癌改良根治术后即刻乳房再造治疗。比较两组患者术后生活质量及远期预后情况。结果术后1年,两组FACT-B评分均升高,且观察组高于照组(P<0.05)。两组局部复发率、远处转移率及无瘤生存率比较,差异不具有统计学意义(P>0.05)。结论保留乳头乳晕乳腺癌改良根治术后即刻乳房再造能够有效提高乳腺癌患者术后生存质量,且不增加远期复发、转移风险,值得推广。

Objective To investigate the effects of immediate breast reconstruction after modified radical mastectomy with nipple-areola preservation on quality of life and long-term prognosis of patients.Methods A total of 280 patients with breast cancer admitted in our hospital from March 2006 to March 2016 were selected as the research objects.According to the random number table method,the patients were divided into control group and observation group,with 140 cases in each group.The control group was treated with modified radical mastectomy with nipple-areola preservation,while the observation group was treated with immediate breast reconstruction after modified radical mastectomy with nipple-areola preservation.The quality of life and long-term prognosis were compared between the two groups.Results One year after operation,the FACT-B scores of both groups increased,and that in the observation group was higher than the control group(P<0.05).There were no significant differences in local recurrence rates,distant metastasis rates and tumor-free survival rates between the two groups(P>0.05).Conclusion Immediate breast reconstruction after modified radical mastectomy with nipple-areola preservation can effectively improve the quality of life of breast cancer patients with breast cancer,and does not increase the risk of long-term recurrence and metastasis,which is worthy of promotion.
