B超评估辅助三孔LC在既往腹部病史患者中的应用效果

Application effect of B-ultrasound assisted three-port LC in patients with previous abdominal history

目的探究B超评估辅助三孔腹腔镜胆囊切除术(LC)在既往腹部病史患者中的应用效果。方法回顾性分析存在既往腹部病史且行三孔LC的120例患者的临床资料,并分析应用B超评估辅助三孔LC术的治疗效果。结果120例既往存在腹部病史的患者中,117例顺利进行三孔LC,手术成功率为97.50%,手术中途转为四孔LC或开腹手术者3例(2.50%);患者手术时间为(43.78±9.23)min,术中出血量为(23.78±5.32)mL,术后住院时间(3.84±0.67)d;术后VRS评分(2.35±0.52)分。术后仅2例(1.67%)患者并发胆汁漏。术前B超影像判断、术中判断结果基本相符。结论B超辅助三孔LC术治疗既往腹部病史患者的效果良好,术前判断粘连情况有利于成功建立气腹,安全性高且创伤小,值得临床推广使用。

Objective To explore the effect of B-ultrasound assisted three-port laparoscopic cholecystectomy(LC)in patients with previous abdominal history.Methods The clinical data of 120 patients with previous abdominal history who underwent three-port LC were retrospectively analyzed.The therapeutic effect of three-port LC assisted by B-ultrasound were analyzed.Results Among 120 patients with previous abdominal history,117 underwent three-port LC successfully,with the success rate of 97.50%.There were 3 cases(2.50%)of conversion to four-hole LC or laparotomy.The operation time was(43.78±9.23)min,the bleeding volume was(23.78±5.32)mL,the hospital stay was(3.84±0.67)d,and the VRS score was(2.35±0.52).The bile leakage only occurred in 2 cases(1.67%).The results of preoperative B-ultrasound and intraoperative judgment were basically consistent.Conclusion B-ultrasound assisted three-port LC in patients with previous abdominal history has a good therapeutic effect,preoperative judgment of adhesion is conducive to the successful establishment of pneumoperitoneum,it has high safety,with minimally invasive,which is worthy of clinical promotion and application.
