累积高敏C反应蛋白与臂踝动脉脉搏波传导速度的关系

The relationship between cumulative high-sensitivity C-reactive protein and brachial-ankle pulse wave velocity

摘要 目的探讨累积高敏C反应蛋白(cumCRP)与臂踝动脉脉搏波传导速度(baPWV)的关系。方法本研究对连续参加了2006-2007、2008-2009、2010-2011年3次健康体检并完成baPWV检测的开滦职工15 505例进行分析,年龄18~89(45.6±11.2)岁,其中男性10 077例(65.0%)。根据cumCRP四分位数将观察对象分为四组:cumCRP<2.90 mg/(L·年)为Q1组,cumCRP 2.90~<5.45 mg/(L·年)为Q2组,cumCRP 5.45~<10.20 mg/(L·年)为Q3组,cumCRP≥10.20 mg/(L·年)为Q4组。采用多因素Logistic回归方法,分析cumCRP对baPWV≥1400 cm/s检出率的影响。结果 15 505例观察对象中,Q1、Q2、Q3、Q4各组平均baPWV分别为1429.32、1523.86、1562.81、1648.10 cm/s(P<0.001)。随着cumCRP的增加,baPWV≥1400 cm/s的检出率逐渐增加(分别为45.38%,57.58%,63.26%和71.77%;P<0.001)。以Q1组做对照的多因素Logistic回归中,校正其他混杂因素后发现,cumCRP是baPWV≥1400 cm/s的独立危险因素,随着cumCRP增加,baPWV≥1400 cm/s的危险增加,Q2、Q3、Q4组的OR值分别为1.26(95%CI 1.13~1.40),1.39(95%CI 1.24~1.55),1.74(95%CI 1.55~1.96)。结论 cumCRP是baPWV增加的独立危险因素。 Objective To investigate the correlation between cumulative high-sensitivity C-reactive protein(cumCRP) and brachial-ankle pulse wave velocity(baPWV). Methods A total of 15 505 individuals who participated three health examinations in 2006-2007, 2008-2009, 2010-2011 and completed the baPWV test were enrolled, in which 10 077(65.0%) were male. The age ranged from 18 to 89 years, with mean age of(45.6±11.2) years. The individuals were divided into four groups according to the cumCRP quartile: cumCRP<2.90 mg/(L·year) for Q1 group, cumCRP 2.90-<5.45 mg/(L·year) for Q2 group, cumCRP 5.45-<10.20 mg/(L·year) for Q3 group, and cumCRP≥10.20 mg/(L·year) for Q4 group. Multivariate logistic regression was used to analyze the effect of cumCRP on the detection rate of baPWV≥1400 cm/s. Results Among 15 505 participants, the average baPWV in Q1, Q2, Q3 and Q4 group were 1429.32, 1523.86, 1562.81 and 1648.10 cm/s, respectively(P<0.001). The incidence of baPWV≥1400 cm/s increased with the rise of cumCRP(45.38%, 57.58%, 63.26% and 71.77%, respectively;P<0.001). Logistic regression analysis showed that cumCRP was an independent risk factor for baPWV≥1400 cm/s and higher cumCRP was a risk factor for increased baPWV after adjustment for other risk factors, OR was 1.26(95% CI 1.13-1.40),1.39(95% CI 1.24-1.55) and 1.74(95% CI 1.55-1.96), respectively. Conclusions Higher cumCRP is associated with increased baPWV.
