耳穴贴压对初产妇围手术期应激反应的影响

Effect of Auricular Point Sticking on Stress Reactions in Primiparae During Perioperative Period

目的观察耳穴贴压对初产妇围手术期应激反应的影响.方法选取择期行剖宫产初产妇200例,按住院先后顺序随机分为观察组和对照组,每组100例.两组均予常规护理,观察组于术前1 h和术后3 h、6 h、12 h及24 h分别给予耳穴贴压干预,随后对比两组的焦虑自评量表、心率、收缩压、促甲状腺激素、皮质醇及血糖水平等.结果最终共189例初产妇完成本次临床研究,其中观察组96例,对照组93例.入室(手术室)时两组焦虑自评量表结果均比入组时明显改善(P<0.05),且观察组改善优于对照组(P<0.05).观察组在入室时和气管拔管时的心率与收缩压均较同期对照组平稳(P<0.05).观察组在入室时、手术开始1 h及术后3 h时的血液促甲状腺激素、皮质醇以及血糖明显优于同期对照组水平(P<0.05),且术后6 h、12 h和24 h的视觉模拟评分也显著低于同期对照组(P<0.05).结论耳穴贴压能减少初产妇围手术期应激反应,促使其心理、生理及情绪的完美协调,是一种良好的分娩辅助疗法.

Objective To observe the effect of auricular point sticking on stress reactions in perimiparae during perioperative period. Method Two hundred primiparae who were going to receive Caesarean section were randomized into an observation group and a control group according to their admission sequence, with 100 cases in each group. The two groups were both given routine nursing care. The observation group was offered auricular point sticking 1 h before and respectively 3 h, 6 h, 12 h and 24 h after the surgery. The Self-rating Anxiety Scale (SAS) score, heart rate, systolic blood pressure, thyroid stimulating hormone (TSH), cortisol, and blood glucose levels of the two groups were then compared. Result A total of 189 primiparae finished the trial in the end, including 96 in the observation group and 93 in the control group. The SAS evaluation was improved at entering the operation room compared with that at enrollment in both groups (P<0.05), and the improvement in the observation group was more significant than that in the control group (P<0.05). The heart rate and systolic blood pressure were more stable in the observation group at entering the operation room and extubation compared with those in the control group (P<0.05). The levels of TSH, cortisol and blood glucose in the observation group were superior to those in the control group at entering the operation room, 1 h during the operation and 3 h following the operation (P<0.05), and the VAS scores at 6 h, 12 h and 24 h after the operation were significantly lower in the observation group than in the control group (P< 0.05). Conclusion Auricular point sticking can reduce the stress reactions in primiparae during perioperative period, and help them harmonize from the psychological, physiological and emotional perspectives. Thus it is a satisfactory assistant therapy for delivery.
