肩关节镜下肩袖修复术治疗肩袖损伤的临床疗效分析

Clinical Analysis of Shoulder Arthroscopy for Rotator Cuff Injury in the Treatment of Rotator Cuff Injury

目的探讨肩关节镜下肩袖修复术治疗肩袖损伤的临床疗效。方法方便选取2016年8月—2018年3月到该院接受治疗的42例肩袖损伤患者,随机分为对照组21例(开放式肩袖修复术)与观察组21例(肩关节镜下肩袖修复术),比较两组住院时长、术后引流量、切口长度、出血量、手术时长、UCLA评分(美国加州大学肩关节评分系统)、ASES评分(美国肩肘外科协会评分)、VAS评分(视觉模拟评分法)、外展90°位外旋、外展90°位内旋、体侧外旋、外展、前屈范围及总有效率指标。结果治疗结束,观察组在住院时长、术后引流量、切口长度、出血量及VAS评分指标(各时段)上均低于对照组(P<0.05);观察组在各时段UCLA评分、ASES评分指标上均高于对照组(P<0.05);观察组在手术时长指标上与对照组差异无统计学意义(P>0.05);外展90°位外旋、外展90°位内旋、体侧外旋、外展、前屈范围指标上均高于对照组且差异有统计学意义(P<0.05);观察组总有效率(96.77%)指标高于对照组(83.87%)且差异有统计学意义(χ^2=2.9524,P<0.05)。结论肩关节镜下肩袖修复术方法可有效改善肩袖损伤患者临床症状,值得推广使用。

Objective To investigate the clinical effect of shoulder arthroscopy in the treatment of rotator cuff injury.Methods 42 patients with rotator cuff injury who were treated in our hospital from August 2016 to March 2018 were randomly divided into the control group(n=21)(open rotator cuff)and the observation group(n=21)(lower rotator cuff repair),comparing the length of hospital stay,postoperative drainage,length of incision,amount of bleeding,length of surgery,UCLA score(University of California shoulder joint scoring system),ASES score(American College of Shoulder and Elbow Surgery score),VAS Scoring(visual analog scoring),abduction of 90°external rotation,abduction of 90°internal rotation,body lateral external rotation,abduction,flexion range and total effective rate.Results At the end of treatment,the length of hospital stay,postoperative drainage,length of incision,blood loss and VAS scores(times)were lower in the observation group than in the control group(P<0.05).The observation group had UCLA score and ASES at each time higher than the control group(P<0.05);The difference between the observation group and the Control group in Dperation tine was not statistically significant(P>0.05);the abduction 90°external rotation,abduction 90°internal rotation indexes of external rotation,abduction and flexion of the body side were higher than those of the control group and the difference was statistically significant(P<0.05).The total effective rate of the observation group(96.77%)was higher than that of the control group(83.87%).The difference was statistically significant(χ^2=2.952 4,P<0.05).Conclusion Shoulder arthroscopy rotator cuff repair method can effectively improve the clinical symptoms of patients with rotator cuff injury,it is worth promoting.
