开放微创手术与经皮Yuet-bone缝合治疗急性跟腱断裂的临床对比研究

Clinical Comparison of Open Minimally Invasive Surgery and Percutaneous Yuet-bone Suture in the Treatment of Acute Achilles Tendon Rupture

目的:探究与分析开放微创手术与经皮Yuet-bone缝合治疗急性跟腱断裂的临床对比。方法:选取本院38例急性跟腱断裂患者,采取随机数字表法分组,每组各19例。对照组给予开放微创手术治疗,观察组给予经皮Yuet-bone缝合治疗,对比两组患者围术期指标,临床疗效,随访3、6个月时健侧、患侧跟腱长度及AOFAS踝-后足评分。结果:观察组与对照组相比手术时间较短、术中出血量减少、切口长度较短、住院时间较短,临床疗效优良率较高,差异均有统计学意义(P<0.05)。两组患者术后6个月患侧跟腱长度相比于健侧跟腱长度较低,差异有统计学意义(P<0.05)。两组患者术后与术前相比AOFAS踝-后足评分较高,且观察组术后6个月AOFAS踝-后足评分高于对照组,差异均有统计学意义(P<0.05)。结论:经皮Yuet-bone缝合治疗相比于开放微创手术治疗急性跟腱断裂的临床短期疗效更加显著,围术期指标突出,预后较好。

Objective:To explore and analyze the clinical comparison between open minimally invasive surgery and percutaneous yuet-bone suture in the treatment of acute achilles tendon rupture.Method:Thirtyeight patients with acute achilles tendon rupture in our hospital were selected and grouped by random number table,19 cases in each group,the control group was given minimally invasive open surgery,the observation group was given percutaneous Yuet-ipads suture treatment,the perioperative indicators,clinical curative effect and followed up for 3,6 months the healthy side,one side the achilles tendon after an ankle-length and AOFAS score of two groups were compared.Result:Compared with the control group,the duration of surgery was shorter,the intraoperative blood loss was reduced,the length of incision was shorter,the length of hospital stay was shorter,the differences were statistically significant(P<0.05).Compared with the control group,the clinical efficacy was higher in the observation group,the difference was statistically significant(P<0.05).The length of the lateral achilles tendon of the two groups was lower than that of the healthy achilles tendon at 3 and6 months after surgery,the difference was statistically significant(P<0.05).The postoperative AOFAS anklehindfoot scores in the two groups were higher than those of preoperative,and AOFAS ankle-hindfoot scores in the observation group 6 months after surgery were higher than that of the control group,the differences were statistically significant(P<0.05).Conclusion:Percutaneous Yuet-bone suture is more effective than open minimally invasive surgery in the treatment of acute achilles tendon rupture in the short term,with prominent perioperative indicators and better prognosis.
