摘要
目的:探讨高通量血液透析对维持性血液透析终末期慢性肾脏病患者血清学指标的影响。方法:选取2017年1-12月于本院接受维持性血液透析的83例终末期慢性肾脏病患者为研究对象,按随机数字表法分为试验组(n=42)和对照组(n=41)。试验组采用高通量血液透析治疗,对照组采用低通量血液透析治疗,比较两组治疗前和治疗1年后β2微球蛋白(β2-MG)、高敏C反应蛋白(hs-CRP)、血钙、血磷、钙磷乘积、降钙素原(PCT)。结果:治疗前两组β2-MG、hs-CRP比较,差异均无统计学意义(P>0.05),治疗1年后试验组β2-MG、hs-CRP明显低于对照组(P<0.05)。两组治疗前血钙、血磷比较,差异均无统计学意义(P>0.05)。治疗1年后两组血钙比较,差异无统计学意义(P>0.05)。治疗1年后,两组血磷均低于治疗前(P<0.05),且试验组血磷及钙磷乘积低于对照组(P<0.05)。两组治疗前PCT比较,差异无统计学意义(P>0.05),治疗1年后试验组PCT明显低于对照组(P<0.05)。结论:高通量血液透析对维持性血液透析终末期慢性肾脏病患者可明显降低其体内β2-MG、hs-CRP、血磷、钙磷乘积及PCT。
Objective:To study the effects of high-throughput hemodialysis on serological parameters in patients with end-stage chronic kidney disease on maintenance hemodialysis.Method:83 patients with endstage chronic kidney disease received maintenance hemodialysis in our hospital from January to December 2017 were selected as the research objects,and they were divided into experimental group(n=42)and the control group(n=41)according to the random number table method.The experimental group was treated with high-flux hemodialysis,and the control group was treated with low-flux hemodialysis.The β2 microglobulin(β2-MG),high-sensitivity c-reactive protein(hs-CRP),serum calcium,serum phosphorus,calcium phosphorus product and procalcitonin(PCT)were compared between the two groups before and 1 year after treatment.Result:Before treatment,there were no significant differences in β2-MG and hs-CRP between the two groups(P>0.05).After 1 year of treatment,β2-MG and hs-CRP in the experimental group were significantly lower than those in the control group(P<0.05).There was no significant difference in serum calcium and phosphorus between the two groups before treatment(P>0.05).After 1 year of treatment,there was no significant difference in serum calcium between the two groups(P>0.05).After 1 year of treatment,the calcium phosphorus in the two groups were lower than those before treatment(P<0.05).And the calcium phosphorus product and calcium phosphorus in the experimental group were lower than those in the control group(P<0.05).There was no significant difference in PCT level between the two groups before treatment(P>0.05).After 1 year of treatment,the PCT level in the experimental group was significantly lower than that in the control group(P<0.05).Conclusion:High-flux hemodialysis can significantly reduce the levels of β2-MG and hs-CRP,blood phosphorus,calcium phosphorus product and PCT in patients with end-stage chronic kidney disease who were on maintenance hemodialysis.
