术前焦虑对行择期腹腔镜卵巢囊肿剥除术患者麻醉苏醒及术后镇痛的影响

Effects of Preoperative Anxiety on Anesthesia Recovery and Postoperative Analgesia in Patients Undergoing Elective Laparoscopic Ovarian Cyst Ablation

目的:分析术前焦虑对行择期腹腔镜卵巢囊肿剥除术患者麻醉苏醒及术后镇痛的影响.方法:选取2018年6月-2019年2月收治的50例择期腹腔镜卵巢囊肿剥除术患者为研究对象,依据状态-特质焦虑调查表(state-trait anxiety inventory,STAI)评分进行焦虑情况分析并分为高焦虑组与低焦虑组,各25例.分析两组术后疼痛情况与麻醉苏醒情况.结果:高焦虑组术后6、12、24 h疼痛评分分别为(3.95±0.68)、(3.17±0.43)、(2.71±0.34)分均高于低焦虑组的(2.48±0.58)、(2.16±0.37)、(2.05±0.27)分,差异均有统计学意义(t=8.224、8.902、7.601,P<0.05).高焦虑组术后6、12、24 h麻醉用药量均明显高于低焦虑组(t=9.424、10.502、11.751,P<0.05);高焦虑组拔管时间(17.26±6.21)min与出室时间(28.13±4.62)min均明显高于低焦虑组(9.17±2.07)、(16.24±2.46)min,差异均有统计学意义(t=5.166、5.261,P<0.05);高焦虑组麻醉苏醒后并发症发生率高于低焦虑组(字2=4.367,P<0.05).结论:术前焦虑情况较为严重的择期腹腔镜卵巢囊肿剥除术患者会出现麻醉苏醒延迟以及术后镇痛要求提升的问题.

Objective:To analyze the influence of preoperative anxiety on anesthesia recovery and postoperative analgesia in patients undergoing elective laparoscopic ovarian cyst ablation.Method:50 cases of elective laparoscopic ovarian cyst ablation admitted from June 2018 to February 2019 were selected as the study subjects.According to the state-trait anxiety inventory(STAI)score,the anxiety situation was analyzed and they were divided into high anxiety group and low anxiety group,25 cases in each group.The postoperative pain and anaesthesia recovery of the two groups were analyzed.Result:The pain score at 6,12,24 h after surgery in the high anxiety group were(3.95 ± 0.68),(3.17 ± 0.43),(2.71 ± 0.34)scores,higher than(2.48 ± 0.58),(2.16 ± 0.37),(2.05 ± 0.27)scores of the low anxiety group,the differences were statistically significant(t=8.224,8.902,7.601,P<0.05).The dosage of analgesics at 6,12,24 h after surgery in the high anxiety group were significantly higher than those in the low anxiety group(t=9.424,10.502,11.751,P<0.05).The extubation time(17.26± 6.21)min and the out of the room time(28.13 ± 4.62)min in the high anxiety group were significantly higher than(9.17 ± 2.07).,(16.24 ± 2.46)min in the low anxiety group,the differences were statistically significant(t=5.166,5.261,P<0.05).The incidence of postoperative complications in the high anxiety group was higher than that in the low anxiety group(χ2=4.367,P<0.05).Conclusion:Patients with severe preoperative anxiety during elective laparoscopic ovarian cyst ablation can delay anesthesia recovery,and increase postoperative analgesia requirements.
