IMA和cTnI联合检测对早期2型糖尿病患者无症状心肌缺血的诊断价值

Diagnostic Value of Combined Detection of IMA and cTnI in Asymptomatic Myocardial Ischemia in Patients with Early Type 2 Diabetes

目的:研究IMA和cTnI联合检测对早期2型糖尿病患者无症状心肌缺血的诊断价值.方法:选取2018年1-12月肇庆市中医院门诊收治的110例疑诊2型糖尿病冠心病无症状心肌缺血患者作为研究对象,入组患者均接受缺血修饰白蛋白(IMA)和心肌肌钙蛋白I(cTnI)检测,并以冠脉CT血管造影(CTA)为金标准,分别比较单独IMA、单独cTnI以及IMA和cTnI联合检测结果.结果:单独IMA检测与单独cTnI检测的灵敏度、特异度、阴性预测值、阳性预测值和诊断符合率比较,差异均无统计学意义(P>0.05),但均显著低于IMA与cTnI联合检测,差异均有统计学意义(P<0.05).结论:IMA和cTnI联合检测能够显著提升早期2型糖尿病患者无症状心肌缺血检查诊断的灵敏度、特异度、阳性预测值和诊断符合率,可推广应用.

Objective:To study the diagnostic value of combined detection of IMA and cTnI in asymptomatic myocardial ischemia in patients with early type 2 diabetes.Method:A total of 110 patients suspected of having type 2 diabetic coronary heart disease with asymptomatic myocardial ischemia who were admitted to the outpatient department of Zhaoqing Hospital of Traditional Chinese Medicine from January to December 2018 were selected as the research objects,all enrolled patients received ischemia-modified albumin(IMA)and cardiac troponin I(cTnI),and were diagnosed by gold standard of coronary CT angiography(CTA).The results of single IMA,single cTnI and combined IMA and cTnI were compared respectively.Result:There were no significant differences in sensitivity,specificity,negative predictive value,positive predictive value and diagnostic coincidence between IMA and cTnI alone(P>0.05),but they were significantly lower than those of IMA combined with cTnI,the differences were statistically significant(P<0.05).Conclusion:The combined detection of IMA and cTnI can significantly improve the sensitivity,specificity,positive predictive value and diagnostic coincidence rate of asymptomatic myocardial ischemia in patients with early type 2 diabetes mellitus,it can be popularized and applied.
