柴桂干姜汤合左金丸加减治疗反流性食管炎的临床疗效观察

Clinical observation on treating reflux esophagitis with the Chaigui Ganjiang decoction plus Zuojin Wan

目的:观察柴桂干姜汤合左金丸加减治疗反流性食管炎的临床疗效。方法:回顾性分析2017年1月-2018年1月接诊的64例反流性食管炎患者的临床资料,应用双色球法随机分为研究组和对照组,各32例。其中对照组患者给予奥美拉唑联合吗丁啉进行治疗,研究组给予柴桂干姜汤合左金丸加减进行治疗。比较分析两组患者治疗后的临床疗效,症状改善情况,24 h食管氢离子浓度指数(pH)监测结果及不良反应发生情况。结果:治疗6周后研究组患者的临床治疗有效率达96.88%,远远高于对照组的71.87%,差异有统计学意义(P<0.05)。研究组患者的烧心、反酸、恶心呕吐和胃痛或胸骨后痛症状评分均低于对照组,差异有统计学意义(P<0.05)。治疗3、6周后研究组患者的反流次数、pH<4的时间占比均低于对照组,差异有统计学意义(P<0.05)。研究组患者的不良反应发生率为3.12%,远远低于对照组的34.38%,差异有统计学意义(P<0.05)。结论:应用柴桂干姜汤合左金丸加减治疗反流性食管炎临床疗效极佳,明显改善患者症状,降低胃镜检查食管炎分级,改善食管pH,降低不良反应率的发生,值得在临床大力推广。

Objective: To observe the clinical efficacy of the Chaigui Ganjiang decoction (柴桂干姜汤) plus Zuojin Wan (左金丸) on reflux esophagitis. Methods: 64 patients were randomly divided into the study group and control group, 32 cases in each group.The control group was treated with omeprazole with morphine, and the study group was treated with the Chaigui Ganjiang decoction plus Zuojin Wan. Results: After 6 weeks, the efficiency in the study group was 96.88%, much higher than 71.87% in the control group(P<0.05).The scores of heartburn, acid reflux, nausea and vomiting, and stomach pain or retrosternal pain were lower in the study group(P<0.05).After 3 and 6 weeks, the number of reflux and pH<4 in the study group were lower(P<0.05). The incidence of adverse reactions in the study group was 3.12%, much lower than 34.38% in the control group(P<0.05). Conclusion: The Chaigui Ganjiang decoction plus Zuojin Wan on reflux esophagitis was effective, by improving the symptoms, reducing the esophagitis classification of gastroscopy, improving the pH and reducing adverse reactions, and is worthy of promotion in clinical practice.
