跗骨窦小切口结合空心钉内固定和手法整复微创治疗跟骨骨折的疗效分析

Efficacy of tarsal sinus small incision with hollow screw fixation and manual reduction plus minimally invasive treatment on calcaneus fracture

目的:探讨跗骨窦小切口结合空心钉内固定和手法整复微创治疗跟骨骨折的疗效。方法:将本院收治的80例单侧跟骨骨折(SandersⅡ、Ⅲ型)患者随机分为两组,每组40例。对照组采用跟骨外侧L形切口结合钢板内固定治疗,实验组采用跗骨窦微创小切口结合空心钉内固定和手法整复治疗。通过观察患者手术前后跟骨宽度、Bohler角、Gissane角、手术时间、术中出血量、切口长度、切口愈合时间和切口并发症,进行两组疗效比较。结果:两组手术后的跟骨宽度、Bohler角及Gissane角均明显优于治疗前(P<0.05),实验组手术时间、术中出血量、切口长度、切口愈合时间和切口并发症均明显优于对照组(P<0.05)。结论:采用跗骨窦小切口空心钉内固定配合手法整复微创治疗跟骨骨折,具有手术时间短、出血少、切口小、切口并发症少等优点,体现了现代微创手术的优势。

Objective: To explore the curative effect of minimal invasive treatment plus the tarsal sinus small incision with the hollow screw fixation and the manual reduction on calcaneus fracture. Methods: 80 patients with sanders type Ⅱ and Ⅲ by the side calcaneal fractures were randomly divided into two groups, and each group was 40 cases. The control group was treated by the calcaneus lateral L-shaped incision with the plate fixation. The experimental group was treated by the tarsal sinus small incision with the hollow screw fixation the manual reduction.Results: The calcaneus width, Bohler angle, Gissane angle after operation were significantly superior to before operation in two groups(P<0.05).The operative time, bleeding amount in operation, incision length, incision heal time and incision complication incidence in the experimental group was significantly superior(P<0.05). Conclusion: The tarsal sinus small incision with the hollow screw fixation plus the manual reduction is minimal invasive used to treat the calcaneus fracture, with shorter operative time, reducing the bleeding amount, small incision and small incision complication. This reflect the superiority of modern minimal invasive operation.
