中药口服联合足浴治疗糖尿病下肢周围神经病变的99例临床观察

Treating 99 cases of diabetic lower extremity peripheral neuropathy with TCM medicine plus foot bath

目的:探讨中药口服加足浴治疗糖尿病下肢周围神经病变疗效。方法:选择2型糖尿病合并周围神经病变患者99例为研究对象,随机分为治疗组50例,在基本降糖治疗的基础上中药口服(黄芪桂枝五物汤合丹参饮加减)加足浴(黄芪桂枝五物汤合桃红四物汤加减)治疗;对照组49例,采用甲钴胺治疗;评价两组的临床疗效。结果:治疗组总有效率为92.0%,显著高于对照组的75.5%(P<0.05);治疗组症状积分、腓总神经运动传导速度(Motor Nerve Conduction Velocity,MNCV)、感觉传导速度(Sensory Nerve Conduction Velocity,SNCV)改善幅度均有高于对照组,且两者相比较差异均有统计学意义(P<0.05)。结论:糖尿病下肢周围神经病变可考虑给予益气活血化瘀治疗,临床疗效突出,患者临床症状积分、运动神经和感觉神经的传导速度明显改善。

Objective: To investigate efficacy of oral Chinese medicine plus foot bath on diabetic peripheral neuropathy of lower extremity. Methods: 99 patients with type 2 diabetes mellitus combined with peripheral neuropathy were divided into 2 groups. 50 cases in the treatment groups and 49 cases in the control group. The treatment group was given basic hypoglycemic treatment plus the Huangqi Guizhi Wuwu decoction(黄芪桂枝五物汤) plus Danshen Yin(丹参饮) and foot bath with Huangqi Guizhi Wuwu decoction plus Taohong Siwu decoction(桃红四物汤). The control group was treated with methylcobalt. Results: The total efficiency in the treatment group was 92.0%, which was significantly higher than 75.5% in the control group(P<0.05). The improvement of symptom score, MNCV and SNCV in the treatment group were higher than those in the control group(P<0.05). Conclusion: The Yiqi Huoxue Huayu(益气活血化瘀) therapy on diabetic peripheral neuropathy shows good effects, can improve clinical symptom score, motor nerve conduction velocity and sensory nerve conduction velocity.
