摘要 目的比较CT与MRI检查技术在腰椎间盘突出症诊断中临床效果差异.方法选取该院2016年10月—2017年10月期间临床收治的腰椎间盘突症患者,共计102例.全部患者均分别采用CT与MRI检查技术进行临床诊断,将两种检查技术的临床诊断结果与患者手术病理诊断结果进行符合,对两种技术的临床诊断符合率进行比较.结果①采用MRI检查技术检出结果的符合率为95.10%,显著高于采用CT检查技术的符合率78.43%,差异有统计学意义(x^2=12.3365,P=0.0004);②采用MRI检查,椎间盘变性、脊髓变形和硬膜囊受压的检出病例数分别为42例、49例和64例,显著多于CT技术检出的13例、16例和40例,差异有统计学意义(x^2=44.291,P=0.000;x^2=47.868,P=0.000;x^2=29.539,P=0.000);采用CT检查,钙化和椎间盘积气的检出病例数分别为54例和61例,显著多于MRI技术检出的20例和18例,差异有统计学意义(x^2=43.145,P=0.000;x^2=64.494,P=0.000).结论 CT与MRI检查技术在腰椎间盘突出症的诊断过程中均能表现出较好的诊断效果,MRI的整体效果更好,在具体实际过程中应结合患者病情科学选择. Objective To compare the differences of the clinical effects of CT and MRI in the diagnosis of lumbar disc herniation. Methods A total of 102 patients with lumbar disc herniation who were admitted to our hospital from October 2016 to October 2017 were selected. The patients were diagnosed by CT and MRI techniques respectively. The clinical diagnosis results of the two examination techniques were compared with the surgical pathological diagnosis results, and the clinical diagnosis coincidence rates of the two techniques were compared. Results 1.The coincidence rate of MRI examination was 95.10%, which was significantly higher than that of CT examination (78.43%), and the differences were statistically significant(χ^2=12.336 5, P=0.000 4). 2.The number of cases with intervertebral disc degeneration, spinal cord deformation and dural sac compression detected by MRI were 42 cases, 49 cases and 64 cases, respectively, which was significantly more than that detected by CT technique (13 cases, 16 cases and 40 cases), and the differences were statistically significant (χ^2=44.291, P=0.000;χ^2=47.868, P=0.000;χ^2=29.539, P=0.000). The number of cases with calcification and intervertebral disc gas detected by CT examination was 54 cases and 61 cases, which was significantly more than that detected by MRI technique (20 cases and 18 cases), and the differences were statistically significant(χ^2=43.145, P=0.000;χ^2= 64.494, P=0.000). Conclusion CT and MRI techniques have good diagnostic results in the diagnosis of lumbar disc herniation. The overall effect of MRI is better. In the actual process, the method should be scientifically selected according to the patient's condition.
