摘要 目的研究与分析自动管电流调节技术在胸部低剂量CT筛查中的临床价值.方法选取来该院进行健康检查的研究者72例,研究对象选取时间为2017年10月—2018年10月,所有研究者均采取胸部低剂量CT筛选,随后将这72例研究者分为对照组与研究组,每组36例.其中对照组研究者采取固定管电流扫描方案处理,而研究组研究者采取固定噪声指数与ATCM对比分析,随后观察两组研究者的图像标准差值(SD)、肺部图像质量评分(IQS)以及加权CT剂量指数(CTDIw)等情况.结果研究组与对照组研究者在肺尖时的SD、IQS等指标差异显著,前者[(13.78±2.32),(1.82±0.33)]显著低于后者[(17.19±2.48),(2.34±0.57)],差异有统计学意义(t=6.0247、4.7370,P<0.05).但是两组研究组在中肺以及下肺时的SD、IQS等指标差异无统计学意义[(11.79±0.88),(11.89±1.28),(1.56±0.36),(1.67±0.73)vs(11.23±2.19),(12.49±2.34),(1.68±0.45),(1.76±0.87)],差异无统计学意义(t=1.4236、1.3497、1.2494、0.4755,P>0.05).研究组与对照组体质量指数超过27 kg/m^2的研究者的CTDIw值差异显著,前者(3.67±0.87)高于后者(2.06),差异有统计学意义(t=11.1034,P<0.05).结论采用胸部低剂量CT筛查健康体验时,可以采取自动管电流调节技术进行处理,要想获得高质量的图像,可以选取体质量较大和大密度衰减范围人群,从而获得较为科学有效的数据,值得进一步学习与借鉴. Objective To study and analyze the clinical value of automatic tube current regulation in low-dose CT screening of the chest. Methods Seventy-two investigators who came to our hospital for health examination were selected. The subjects were selected from October 2017 to October 2018. All the investigators used chest low-dose CT screening, and then the 72 patients were divided into the control group and the study group, with 36 cases in each group. The control group was treated with a fixed-tube current scanning protocol, while the comparison and analysis of fixed noise figure and ATCM were used in the study group. And then, the image standard deviation (SD), lung image quality score (IQS) and the weighted CT dose index (CTDIw) of the two groups were observed. Results The SD, IQS of the researchers in the study group and the control group at lung tip were statistically significany different. The data of the former were [(13.78±2.32), 1.82±0.33)], which were significantly lower than the latter [(17.19±2.48),(2.34±0.57)](t=6.024 7, 4.737 0, P<0.05). However, in the middle lung and lower lung, there was no statistically significany difference in SD, IQS between the two groups[(11.79±0.88),(11.89±1.28),(1.56±0.36),(1.67±0.73) vs (11.23±2.19),(12.49±2.34),(1.68±0.45),(1.76±0.87)](t=1.423 6, 1.349 7, 1.249 4, 0.475 5, P>0.05). The CTDIw values of the researchers with body mass index over 27 kg/m^2 in the study group and the control group were statistically significany different. The former (3.67±0.87) was higher than the latter (2.06), with statistical significance (t=11.103 4, P<0.05). Conclusion When using the chest low-dose CT to screen healthy subjects, automatic tube current regulation technology can be used for processing. To obtain high-quality images and scientific and effective results, people with large body mass and large density attenuation range can be selected. The data is worth learning.
